Care coordinators provide extra time, capacity, and expertise to support patients in preparing for or following up on clinical conversations they have with primary care professionals. They work closely with GPs and other primary care staff within the surgery to identify and manage a caseload of patients, ensuring appropriate support is available to them and their carers, and addressing their changing needs. They focus on delivering the comprehensive model to reflect local priorities, health inequalities, or population health management risks as required.
This role is officially part of Ribblesdale PCN, but the successful candidate will work at Sabden and Whalley practices only and will be under the HR care of the respective practice.
Main duties of the job
* Proactively identify and support a cohort of people with their personalized care needs, using available decision support aids.
* Collaborate with the data quality lead to contact and invite patients for annual reviews using the Ardens system.
* Assist patients in managing their needs, answering queries, and supporting appointment scheduling.
* Ensure patients receive necessary vaccines and participate in annual vaccine campaigns.
* Provide patients with quality information to help them make informed choices about their care.
* Work with safeguarding professionals to complete safeguarding forms and case conference paperwork.
* Contribute towards local enhanced services under the guidance of the data quality lead.

Our team includes 5 partners, 6 salaried GPs, an ANP, 5 nurses, 2 HCAs, and a large reception and admin team. We are a longstanding GP training practice with a housebound nurse team supporting care home and housebound patients. Our PCN team includes a pharmacist, pharmacy technicians, and a physiotherapist. We also have onsite dispensaries at both sites to ensure high-quality patient experiences. Our team collaborates well, making the surgery a harmonious place to work. Experience
* Willingness to undertake care coordination training if not already qualified.
* Experience working in a primary care environment.
* Experience in continued professional development.
* NHS/primary care general practice experience.
