Specialist Nurse – Community Palliative Care Team
There is an exciting opportunity for a band 6 Specialist Nurse within the Community Palliative Care Team. The team provides specialist palliative care services to Oxfordshire and South Northamptonshire, including home visiting, outpatient and Attend Anywhere clinics and telephone advice. We offer a 7‑day service that includes proactive support, a telephone service and a Virtual Ward. Weekend and extended hours working will be required for this post. We are looking for candidates who display excellent communication skills in supporting patients and their families and strong leadership qualities. Palliative or End of Life Care experience is essential.
Main duties of the job
As a specialist nurse within the team, the post holder will demonstrate patient‑focused expert clinical assessment and judgement, develop autonomous and multi‑disciplinary team working, and hold a full driving licence and car ownership. The post holder will also support end‑of‑life care, symptom management and communication skills training for all levels of staff.

Experience
* Demonstrates knowledge or transferrable skills within palliative care and community care
* Experience of clinical governance practices, audit, service evaluation or quality improvement in clinical practice
Skills and abilities
* Commitment to evidencing professional development, capabilities and competency through work‑based assessments and portfolio use
* Advanced communication skills training
