This is an exciting opportunity for a Specialised (Band 7 to 8a progression) or a Highly Specialised Renal pharmacist (8a) to join a dynamic renal pharmacy service for the Wessex Kidney Centre (WKC). You will be appointed to the grade according to your knowledge, skills and experience. Where you are recruited to a Band 7 position we will support your progress to an 8a role based on the Advanced Level Framework. The successful candidate will work within a well-established team with an excellent culture of multidisciplinary working, providing opportunities to develop and embed a career in renal pharmacy whilst delivering a positive impact on patient experience.
PHUT has a large tertiary Renal and Transplant Unit with a catchment population of 2.2 million patients. The successful applicant will join the specialist renal pharmacy team, who provide the Renal Transplant Homecare service and pharmacist cover for the renal wards along with provision of pharmaceutical services to satellite dialysis units.
There is an excellent culture of multidisciplinary teamworking and there are opportunities within the role to run clinics and become involved in research. You will have the opportunity to make a significant difference to clinical outcomes and patient safety, providing medicines optimisation to a complex cohort of patients.
Main duties of the job
Key Responsibilities:
* The successful overall performance of the Trust.
* Act as an advocate for the Trust & its contribution to the Health Service arena through creating and maintaining effective partnerships and relationships with staff, patients and the general public.
* Develop and support the Trust culture of collaborative, flexible cross-team working and commitment to delivering quality services and outcomes, which maintain the Government's policies on public health.
* Work with sensitivity and an understanding of the issues facing those working to deliver health services to the UK population.
* Comply with corporate governance structure in keeping with the principles and standards set out by the Trust.
* Provide highly specialised clinical pharmacy expertise in use of medicines for patients within Renal Services during an inpatient hospital stay or on an outpatient or day case basis.
* Monitor the performance, efficiency and quality of the renal pharmacy service.
* Provide clinical support to the renal anaemia and immunosuppression homecare service including generation and clinical screening of homecare prescriptions.
* Undertake the role of non-medical prescriber, with the appropriate qualifications according to the needs of the service.
* Monitor patients on anaemia treatment to ensure individual optimisation within the appropriate target range.

Job responsibilities
Job Summary for Band 7 & Band 8A:
* To be a Specialist Clinical Pharmacist for Wessex Kidney Centre (WKC) utilising specialist clinical knowledge to provide a high-quality pharmaceutical service to patients.
* To practice as a non-medical prescriber within the renal pharmacy service with specific expertise of renal medicines.
* To provide highly specialised clinical pharmacy services to WKC to optimise patient benefit from treatment and ensure safe, efficacious, cost-effective use of medicines.
* Play an integral role in supporting the MDT in the development and day to day running of the renal service.
* Respond and action clinical queries regarding renal patient management.
* Development of comprehensive renal guidelines and generation of patient supportive materials.
* Work with key stakeholders across the WKC region to ensure the effective and safe delivery of the renal pharmacy service.
* Provide expert advice on pharmaceutical matters when carrying out clinical pharmacy services.
* Clinical screening and generation of renal homecare prescriptions.
* To be an integral member of the highly specialised pharmacy renal team that provide a clinical service to the Wessex Kidney Centre (WKC).
* To provide pharmaceutical advice and develop services to optimise patient benefit from treatment with medicines and ensure the safe, efficacious and cost-effective use of medicines.
* To undertake Risk Management and ensure compliance with medicines legislation.
* To support the development and expansion of the Renal Pharmacy service.

Person Specification
Qualifications
* Masters or Bachelors degree in pharmacy.
* Registered Pharmacist with General Pharmaceutical Council.
* Completed or studying for Diploma/MSc in Clinical Pharmacy.
* Member of Royal Pharmaceutical Society.
* NMP qualification or working towards and eligible for training.
* Member of specialist pharmacist practice group e.g. UKCPA.
* Completed or studying for diploma in Clinical Pharmacy or equivalent.
* Independent Prescriber qualification or working towards or eligible for training.
Experience
* Minimum 3 years pharmacy experience in clinical pharmacy services.
* Previous experience as a pharmacist in a leading role (team, service or directorate responsibilities).
* Experience in working in a multidisciplinary team.
