We are seeking a motivated and compassionate Nurse Practitioner to join our team. The ideal candidate will be responsible for providing comprehensive care to our patients, working collaboratively with our multidisciplinary team.

Key Responsibilities


* Practice autonomously within scope using a person-centred, partnership-based approach.
* Assess and diagnose individuals accurately during acute and long-term health episodes.
* Gather and evaluate clinical data to make informed, evidence-based decisions. Carrying out referrals if necessary.
* Coordinate and manage care plans to optimise health outcomes and resource use.
* Provide emotional support and reassurance to individuals, carers, and families.
* Order, carry out and interpret tests such as blood work, imaging, or screenings to support diagnosis and treatment planning.
* Apply sound clinical judgment and use up-to-date research to guide decisions and care.
* Deliver health promotion and disease prevention advice, including support for smoking cessation, weight management, and contraception.
* Manage long-term conditions by providing patient education.
* Support minor illness and injury management, including prescribing and treatment planning within scope of practice.
* Use appropriate screening tools to confirm diagnoses and guide treatment.
* Contribute to multidisciplinary team meetings and collaborate with GPs, pharmacists, and allied health professionals to ensure coordinated care.

Qualifications

Essential

* Registered with the Nursing and Midwifery Council (NMC) to practice as a Registered Nurse
* Completed an NMC-approved nursing degree (BSc or Nursing Degree Apprenticeship)
* V300 Independent Prescribing Qualification.
* Evidence of a commitment to on-going personal & professional development

* Postgraduate Diploma or Certificate in a Specialized Area
* Teaching and Mentorship Qualification
