Overview
To work under the guidance and supervision of qualified members of staff to meet the needs of the clients in their own home. The job may also occasionally require the provision of care of clients in some other primary health care setting. Calderdale provides a 24 hours district nursing service and the job entails supporting this service. The post is for an out of hours healthcare assistant working between the hours of 19:00 - 08:00, the shifts can and will fall on any day in a week. Based at Dean Clough Halifax
Requirements
* A valid UK driving licence is essential for the post with access to your own vehicle
Responsibilities
* Assist in the delivery of unscheduled, unpredictable episodes of care on a regular basis, with appropriate use of initiative.
* Be willing to undertake any additional training appropriate for this role, to enable effective delivery of care of the client group.
* Support the community nursing service as part of Calderdale & Huddersfield NHS Foundation Trust (CHFT).
* Manage information and documentation in line with Trust policies and procedures.
Qualifications and attributes
* Motivated and passionate about high quality patient care with excellent interpersonal skills; flexible and able to work under pressure.
* Previous experience in a healthcare setting is desirable.
* Essential: confident, resilient and caring nature.
* Able to create a professional nursing culture open to change, new ideas, concepts and innovation whilst reflecting organisational strategy and values.
Training and development
* Willing to develop new skills and participate in training in line with service development, e.g. venepuncture, administration of defined medicines, enteral feeding.
* Has an understanding and knowledge of community care procedure and practice (basic observations, catheter care, monitoring of blood sugar/blood pressure etc).
* Able to participate in appraisal and align personal objectives with organisational aims.
Managing Information
* Use appropriate information to make timely and effective decisions in relation to the community nursing team.
* Contribute to the development of information systems and obtain, transmit and store information relating to the delivery of care services (including audits).
* Use information and technology, including inputting data on the computer system to enable the team to use it.
* Provide information in a form that meets the understanding of the receiver at all levels; ensure continuity of patient care throughout the 24-hour period.
* Complete documentation in accordance with Trust’s policies and procedures.
Managing Self
* Participate in regular supervision.
* Attend all mandatory training and participate annually in identifying, developing and agreeing a personal development plan with your Line Manager using the Trust Appraisal.
* Comply with all Trust policies, procedures and protocols; uphold Equal Opportunity Policy.
* Seek advice and support from Line Manager when necessary; maintain professional conduct and appearance at all times.
* Maintain professional registration where applicable.
