Job overview
Intermediate careis a service that helps people during a period of severe or sudden illness, or when they have been discharged from a general hospital, so they can be cared for and recover in their own homes or as an interim in Clayponds Hospital.
1. To provide evidenced based clinical care to patients whilst they recover from a period of poor health or a functional decline and leadership to the MDT.
2. To contribute to and ensure a safe environment for patients, staff and visitors.
3. To ensure the successful implementation of care as prescribed by the MDT or yourself.
Main duties of the job
4. To access, plan and implement care to patients in line with MDT
5. Undertake advanced clinical assessments and clerk patients on admission.
6. To create an environment conducive to learning and ‘no blame’ culture and communicate effectively with all healthcare colleagues, patients and carers.
7. To ensure that documentation meets the NMC standards for record keeping and confidentiality.
8. To ensure that an individual programme of care is planned implemented and evaluated.
9. Work with senior staff to develop and implement care pathways, clinical practice, guidelines and protocol in line with NICE guidance and evidence to deliver quality care.
10. To be accountable for own professional action adhering to The Nursing and Midwifery Code of Conduct.
11. To be readily accessible to each service user.
12. To report to the Ward Manager/Matron and work in line with Trust Policies and protocols.

Qualifications and Training
Essential criteria
13. •Registered General Nurse, Adult Branch and current NMC Registration
14. •Experience and evidence of teaching and assessing e.g. Preceptorship/Mentor
15. •Commitment to complete Independent Nurse Prescriber course
16. •Advanced clinical assessment skills at minimum level 6 study or similar course or commitment to complete.
17. •Evidence of recent CPD

18. •Advanced clinical skills - ability to assess, diagnose and treat as an independent practitioner minimum level 6 study or similar

Experience
Essential criteria
20. •Demonstrable experience and work qualification, preferably within an Intermediate Care or community environment.
21. •Experience of supervising junior staff in the team
22. •Experience of quality improvement processes including audit, risk management and a good understanding of clinical governance and change management
23. •Experience of and ability to communicate effectively with variety of partners stakeholders - verbal, written and electronically and
