We are looking fora Band 6 Named Professional to join our highly skilled MDT based at The LaurelsGender Identity Clinic, in Exeter.
We support Transand non-binary adults to receive treatment tailored to their unique experiencesof gender incongruence, offering specialist gender assessments and treatmentplans, specific gender focused psychological therapy, speech and languagetherapy, specialist medical consultations, and surgical referrals.

Main duties of the job
NamedProfessionals are accredited healthcare professionals with a background in Nursing(Mental Health and/or General), Occupational Therapy, Social Work orCounselling. They act as the patients primary point of contact with the GIC,provide basic information about interventions and the care process and overseeand facilitate timely progress through the patients treatment plan. Theyliaise and share information with other clinicians involved in the patientscare (e.g. Psychology, Voice and Communication Therapy or Specialist MedicalPractitioners) as well as the patients GP. When required, Named Professionalsalso deliver supportive guidance throughout their care.
When necessary,Named Professionals will be expected to liaise with external agencies such asMental Health Teams, Social Services or other specialists (NeurodiversityServices or Eating Disorders). They will also take part in professionalmeetings surrounding clinical care of patients with other members of thepatients team.
Named Professionals are also involved in the assessment of patients as they enter our clinic.

Job responsibilities
Thepost holder will have highly developed communication skills which will include:
* Goodcommunication with service users (and with their agreement, carers andsignificant others.) To communicate in a highly skilled and sensitive mannerwith service user who present to the clinic. This may include people who havespecific difficulties in understanding and/or communicating and who may behighly emotionally.
* Other health professionals as appropriate and multidisciplinary teamcolleagues. To work closely with colleagues in other services, or GPs, andother health and social care staff. Contributing to the teams engagementincluding liaising with a wide range of other professionals and agencies.
* To communicate with a wide range of health & social care staff inother services in a highly skilled and sensitive manner regarding the complexand sensitive information concerning the assessment, formulation and treatmentplans of people under the services care.
* External Agencies such as thePolice, housing, employment etc.
* Contribute to the safe andeffective day to day operations of the service including the handling ofclinical enquiries.
* Provide a high standard ofwritten and verbal communication that is clear and in line with professionaldocumentation standards.
* Promote social inclusionregardless of age, ethnicity, gender, sexual orientation or disability.
* Undertakerisk assessments and determine the appropriate course of action. Whenappropriate will seek consultation with senior clinical leads in theservice.
* Undertake initial assessments and information gathering to informan ongoing diagnostic assessment.
* Undertakeand continuously re-evaluate bio-psycho-social assessments and contribute tothe development of a working formulation and diagnosis, in consultation withsenior clinical staff, and the multi-disciplinary team.
* Havean awareness of your own clinical competencies, knowledge base and experience,and ensure that you update and practice within this, seeking support ifrequired.
* Thepost holder will need to have a wide range of planning and organisationalskills including but not limited to:
* Carryinga specialist caseload of service users, and providing practical advice to them,family members, carers and professionals.
* Organising their own day to day working patternsin a timely and effective way taking the wider needs of the team intoconsideration
* Ensure that interventions are planned in anintegrated and co-ordinated manner, this may include liaison with externalagencies.
* Liaising and planning with other professionalsthat provide all other interventions for people who access our service.
* Mustbe IT competent.
* Organisemeetings, including non-CPA professional and risk meetings.
* The post holder must demonstrate a willingnessto engage in the specialist training in Gender Identity Disorders makingappropriate use of supervision.
* Tomaintain appropriate accurate and up to date client records
* Ensure that clinical practice is developed andmaintained in line with the latest research and evidence base.
* Ensure that designatedcaseload is well managed and that care and treatment is provided in line withpractice standards
* Understand social factorsthat impact upon the mental health of people experiencing genderincongruence
* To consider the holistic needs of service usersand identify actions to address these needs within the context of a jointlyagreed and negotiated care plan.
* Ensure that all interventions have clearoutcomes and that these are taken into consideration when sourcing theresources to best meet the individuals needs.
* Deliver care in line with service protocols,evidence based practice and employ engagement and therapeutic skills that areappropriate to the clinical situation presented.
* Provide specialist advice to referrers/othermulti-disciplinary staff in relation to the care of individuals.
* Enable communication and engagement ofindividuals and their significant others to ensure that they have a voice aboutthe services they receive and how these are developed. This may includeengagement of advocacy services.
* Ensure that appropriate action is taken tosafeguarding adults and children. This may include contributing to safeguardingmeetings.
* Ensuring timely and accurate documentation ofall clinical activity in line with professional guidance and bestpractice.
* Practicein a way that actively minimises dependency and promotes well-being.
* Contribute to clinicalgovernance arrangements in order to ensure the quality of the service provided,this will include ensuring performance targets are met.
* Thepost holder will be responsible for their own professional actions ensuring:
* Engagementin the development of Protocols for the gender service as guided by linemanagement
* Involvementin the implementation of local systems and process in your own area ofwork
* That they follow the policiesof the Devon Partnership NHS Trust and all other relevant polices.
* That they are working withinthe standards expected by the clinic in relation to working with clients andreferring clients to other services as appropriate.

Person Specification
Qualifications
* An appropriate up to date professional mental health qualification eg RMN, OT or social worker on the appropriate professional body register and hold current NMC, Social Work Council or HCPC registration with no restrictions - (It will be an essential requirement for newly qualified social workers to demonstrate that they have completed The Assessed and Supported Year in Employment (ASYE) before commencing in post or there will be a requirement to complete this within the first 12 months of employment with the Trust).
* Evidence of relevant continuing professional development. Training in gender and sexual diversity.
* Training to level 2 in Safeguarding children and young people: roles and competencies for Health Care staff
* Post registration training in relevant field

* Managing a caseload of clients with a range of problems.
* Demonstrates sound theoretical knowledge of mental health practice
* Experience of working with issues of difference, diversity and social inequality.
* Experience of receiving and providing clinical supervision.
