As a Senior Social Worker, you’ll work in close collaboration with families, health, education, and social care professionals to provide strengths-based support. What you will do
* Lead direct practice by conducting comprehensive family help discussions and devising tailored intervention plans.
* Utilize your experience with children with disabilities to perform assessments, care planning, and crisis interventions.
* Work to reduce the escalation of families’ needs by implementing targeted strategies.
* Drive multi-agency coordination by chairing planning meetings that deliver holistic, child-focused support.
* Apply your knowledge of legislative and safeguarding frameworks effectively.
* Implement the Pathfinder’s graduated model focusing on early intervention.
* Prepare young people with SEND for adulthood through transition-focused initiatives.
* Engage in reflective supervision and team learning to enhance practice and outcomes.
