In this role, you will conduct comprehensive assessments and deliver psychosocial, evidence-based interventions to individuals experiencing severe and enduring mental health challenges, supporting them on their path to recovery. Key Responsibilities
* Deliver a high-quality, comprehensive mental health service to individuals with complex and enduring needs within the community.
* Manage a defined caseload, carrying out assessment, planning, implementation, and evaluation of care in collaboration with service users and carers.
* Provide psychosocial interventions and support to help service users manage daily living, finances, housing, and community engagement.
* Offer clinical advice, leadership, and supervision to junior staff and students, supporting their professional development.
* Work collaboratively with inpatient units, CMHTs, HTTs, primary care, and partner organisations to ensure continuity of care.
* Promote service user involvement in care planning, reviews, and service evaluation.
* Participate in service development, audit, and quality improvement initiatives.
