Join to apply for the Specialist Occupational Therapist role at Mersey Care NHS Foundation Trust.

Our Southport & Fomby Urgent and Intermediate Care therapy team are seeking committed Band 6 Occupational Therapists to join our service, supporting patients through shared care within their localities. We are seeking highly skilled, enthusiastic, and proactive therapists to join our team. The service responds to the rehabilitation needs of adults 18+, who have multiple complex co-morbidities or require support with discharge from the hospital setting. The Urgent Community Response service (UCR), Care Transfer Hub and Intermediate Care Bed Base services operate 7 days a week including bank holidays. All staff are expected to rotate through the UCR, Care Transfer Hub and Intermediate Care Bed Bases working alongside the full Multidisciplinary Team to cover weekends on a rotational basis.

We thrive to communicate and collaborate between Integrated care systems to address the health and social care needs across the area, maximising patient independence and preventing hospital admissions. This is a fantastic opportunity to join our multi-disciplinary team. The team consists of Physiotherapists, Occupational Therapists, Assistant Practitioners, Health Practitioner Assistants, Community Discharge Matrons, Nursing staff and social workers.

The post holder will contribute to the planning, development and evaluation of OT services within a designated area/team, holding responsibility for defined projects, and will take a holistic approach in the assessment and treatment of service users/patients, providing high quality care to a defined caseload which may be complex, using evidence-based practice.

Work in partnership with other professionals to enable patients/clients to be maintained in an appropriate environment, thus avoiding hospital admission. The post holder will provide leadership for junior staff when appropriate, through supervision and appraisal.

The post holder will contribute to the maintenance and development of the Occupational Therapy profession within ICRAS and will facilitate effective learning within the area of practice for all students and practitioners.
