Mosaic PCN has a growing team of pharmacists and pharmacy technicians who will benefit from a dynamic team leader who can provide support and leadership when required. There are opportunities to upskill and take on projects, as well as being part of a dedicated team. The role covers urban and rural patients so there is variation in the requirements of patients

About us

Mosaic Primary Care Network consists of 3 practices over 5 sites (Crown Heights Medical Centre, Odiham & Old Basing Health Centres and Bramblys Grange Medical Practice) located in or near Basingstoke. We have a combined patient population of circa 60,000 patients. We are looking for a lead pharmacist to join our expanding team.

The PCN is a registered learning environment, so all our ARRS staff are supported by an EELS lead to ensure we meet the aspirations of both the team member and the surgeries.


Job responsibilities

You willneed to work across the PCN practices.

Youmust be a registered pharmacist with experience in Primary Care.

Youmust be a prescriber or currently completing training to prescribe.

Youshould have some experience in leading a team.

You willneed to hold a full driving licence and have access to your own transport.

Job Overview

To improve health outcomesand the efficiency of the primary care network (PCN) pharmacy team by providingdirect, accessible and timely medicines expertise as part of amulti-disciplinary team. To pro-activelycomplete workloads relating to medicines optimisation issues from otherclinical staff to improve patient care, safety and outcomes.

The Lead Clinical Pharmacist must be a prescriber and registeredwith the General Pharmaceutical Council (GPhC). This role requires enrollmentin (if not already qualified from) an approved 18-month training pathway orequivalent that equips the Clinical Pharmacist to be able to practice andprescribe safely and effectively in a primary care setting, for example, theCPPE Clinical Pharmacist training pathways.

The summary of the role includes:

a. work aspart of a multi-disciplinary team in a patient facing role to clinically assessand treat patients using their expert knowledge of medicines for specificdisease areas;

b. be aprescriber, or completing training to become a prescriber and work with andalongside the general practice teams;

c. beresponsible for the care management of patients with chronic diseases andundertake clinical medication reviews to proactively manage people with complexpolypharmacy, especially older people, people in care homes, those withmultiple co-morbidities (in particular frailty, COPD and asthma) and peoplewith learning disabilities or autism (through STOMP Stop Over MedicationProgramme)

d. Provide specialist expertise in the use of medicines whilst helping to address both thepublic health and social care needs of patients at the PCNs practices and tohelp in tackling inequalities;

e. provideleadership on person-centered medicines optimisation (including ensuringprescribers in the practices conserve antibiotics in line with localantimicrobial stewardship guidance) and quality improvement, whilstcontributing to the quality and outcomes framework and enhanced services;

f. throughstructured medication reviews, support patients to take their medications toget the best from them, reduce waste and promote selfcare;

g. have a leadership role in supporting furtherintegration of general practice with the wider healthcare teams (includingcommunity and hospital pharmacy) to help improve patient outcomes, ensurebetter access to healthcare and help manage general practice workload;

h. develop relationships and work closely with otherpharmacy professionals across PCNs and the wider health and social care system;

i. take a central role in the clinical aspectsof shared care protocols, clinical research with medicines, liaison withspecialist pharmacists (including mental health and reduction of inappropriateantipsychotic use in people with learning disabilities), liaison with communitypharmacists and anticoagulation; and

j. be part ofa professional clinical network and have access to appropriate clinical supervision. Each clinical pharmacist must receivea minimum of one supervision session per month by a senior clinical / advancedpractice pharmacist;

k. the senior clinical / advanced practicepharmacist must receive a minimum of one supervision session every three monthsby a GP clinical supervisor;

l. each clinical pharmacist/senior clinicalpharmacist/advanced practice pharmacist will have access to an assigned GPclinical supervisor for support and development;

m. a ratio of one senior clinical / advancedpractice pharmacist to no more than five junior clinical pharmacists, withappropriate peer support and supervision in place.

n.undertakeclinical audits as agreed with the prescribing lead;

o.receive and resolve medicine queries frompatients over the telephone and/or face to face,

p. lead and coordinate the PCN pharmacy team and be a point of accessfor any support they may require such as meetings, educational needs,professional discussions,

q. beresponsible for workflow optimisation and delivery of daily services,

r. coordinate& deliver appraisals for the pharmacy team,

s. work alongside GP practice staff as anintegrated part of the practice teams.

Responsibilities underpinning the role

To develop and facilitate a good working relationship with alllocal providers of healthcare.

To work closely with the Prescribing Leads andPractice Managers to agree job objectives and timetable

To recordpersonally generated information and maintain a database of informationrelating to the work done in the practice(s). Personally generated informationincludes information and records relating to audit and clinical work undertakenby the post holder, reference notes relating to clinical/technical information.

Maintain recordsof all work done in the practice for which the post holder is accountablethrough the correct use of the clinical system.

To maintainregistration as a pharmacist and comply with appropriate professional codes.

As appropriate tothe post, to maintain and develop professional competence and expertise, keepup to date with medical/therapeutic evidence and opinion, local and nationalservice specifications, legislation and policy developments,

To agreeobjectives and a personal development plan and participate in the appraisalprocess.

To attend local,regional and national meetings of relevance as required by the practice.

To undertake anyother duties commensurate with the post holders grade as agreed with the postholders line manager.

Person Specification


Qualifications

* Must be registered with the General Pharmaceutical Council.
* Must be a prescriber or currently working towards this.
* Experience leading a team is needed.
* Must be prepared to work from more than one site.
* Driving licence is needed due to the rural location of some sites.
* Must have completed the CPPE primary care pathway ( or equivalent) or be wiling to complete this.
* Experience leading a team is desirable.
* Experience in primary care is desirable.
