Job summary
We are looking for an Advanced Pharmacist to deliver a highly specialist pharmacy service to the adult oncology & haematology ward and the acute emergency assessment unit. We require an active member of the multidisciplinary team who promotes the safe, appropriate and clinically effective use of medicines.
Main duties
* Provide a Specialist Clinical Pharmacy Service to adult oncology & haematology ward and the acute emergency assessment unit
* To lead, develop and provide a specialist clinical pharmacy service to the adult oncology & haematology ward and the acute emergency assessment unit
* To work closely with the oncology & haematology clinicians to implement and proactively promote and develop safe, high quality and evidence-based prescribing and safe management of medicines
* To communicate highly complex information relating to medicines with clinicians, nurses, independent prescribers, carers and patients
* To act as a highly specialised clinical pharmacist and provide a patient focused clinical pharmacy service on the adult oncology & haematology ward and the acute emergency assessment unit including proactive and reactive participation in ward rounds, input into medicines management review, counselling, acting as a clinical pharmacy role model and being accountable for own professional actions
* To develop and evaluate the delivery of a responsive, proactive, patient-focused Clinical Pharmacy Service to patients receiving treatment for cancer

Job responsibilities
The main responsibilities of the post will include the following:
Specialist Clinical Pharmacy Service to Adult Clinical Oncology and Haematology - inpatient, emergency assessment unit, outpatient and day case units for haematology and oncology
* Deliver a high level Specialist Clinical Pharmacist Service to Adult Clinical Haematology and Oncology, including daily clinical pharmacist service to inpatients, outpatient clinics and day case chemotherapy
* Use specialist knowledge to make complex judgements and provide expert clinical advice, evaluating evidence and advising where evidence is lacking
* Work as a prescriber in haematology clinics
* Participate in daily ward and board rounds, attend MDTs, advise and train junior staff
* Provide general and specialist medicines information to health professionals, managers and patients
* Provide pharmaceutical support to manage medicines safely and cost-effectively
* Plan audits and practice research; implement service changes to Adult Haematology and Oncology
* Lead development and implementation of medicine-related guidelines, policies and procedures
* Communicate appropriate clinical interventions and discuss with the Cancer Services Team
* Support incident and complaint investigations related to medicines usage
* Adhere to local and national guidance and policies
* Supervise, check and undertake supply of medicines as needed
* Optimise medicines through prescribing within authorised areas
* Maintain up-to-date clinical knowledge and fulfill the Code of Ethics and Duty of Candour

Person specification
Educations, Training and Qualifications
* Essential: Masters degree in Pharmacy; Registered with GPhC; Diploma or MSc in Clinical Pharmacy; Independent Prescriber; Mandatory CPD Portfolio; Member of BOPA
* Desirable: Formal management training; Member of Royal Pharmaceutical Society; Working towards RPS faculty recognition; Advanced Clinical Practitioner qualification; Diploma Tutor accreditation
Knowedge and Experience
* Essential: Extensive operational experience at a senior hospital pharmacist level; strong knowledge of pharmaceutical care for haematology and oncology; experience with MDTs, formulary, guidelines; teaching and training; practice research or audits; core therapeutics knowledge
* Desirable: Ward round attendance; prescribing experience in speciality; people management; risk/change management; Directorate Pharmacist
Skills
* Essential: Analytical, clinical reasoning, problem solving; strong communication; ability to work under pressure and independently; teamwork; prioritisation; delegation; time management; information appraisal; planning and monitoring; mature organisation; good interpersonal skills
