Job Overview

Integrated Care Pharmacist, Community Independence Service (37.5 hours); Band 8a; 9‑month fixed‑term contract (maternity leave cover). We have an exciting opportunity for a highly motivated, enthusiastic pharmacist with excellent clinical skills to join our Community Independence Service (CIS). The post‑holder will provide a clinical pharmacy service to CIS and lead on medicines optimisation (MO).

The CIS service aims to offer safe care at home, enabling patients to avoid unplanned hospital admissions. The pharmacy role is pivotal in reducing the risk of avoidable admissions due to medication.

Central to your role will be carrying out individual assessments in patients with complex medicines needs or with an identified medication issue. This will require travelling to the patient’s home to carry out the assessment. You will work in close partnership with other members of the CIS team to provide high‑quality patient care.


Main Duties of the Job

* Lead on the delivery of a clinical pharmacy service across Community Independence Service (tri‑borough).
* Be the source of expert clinical advice for medicines information related queries.
* Carry out domiciliary assessments and medication reviews in identified high‑risk patients with complex medicines needs.
* Formulate medicines care plans jointly with the patient and/or carer.
* Communicate interventions with the patient/carer, their GP and other key health professionals involved in their care.
* Provide professional advice and support on the safe prescribing, handling and administration of medicines.
* Collaborate with internal and external stakeholders to improve and develop robust medicines optimisation pathways.
* Manage and provide clinical supervision to pharmacy staff within the designated area.
* Work with GPs and participate in MDTs.
* Lead on service evaluation and monitoring.


Detailed Job Description and Main Responsibilities

The post holder will be responsible for providing, leading and developing a medicines optimisation (MO) service to CIS within Kensington & Chelsea and Westminster boroughs. They may also support and provide MO leadership to the Hammersmith & Fulham CIS team when needed. The overarching aim is to support patients recently discharged from hospital with their medicines to improve safety, adherence to treatment, reduce waste and prevent avoidable readmissions due to medicines. The holder will lead on developing systems to facilitate efficient, safe and cost‑effective use of medicines, working across interfaces to improve MO.


Person Specification


Education and Qualifications – Essential Criteria

* Vocational Masters’ Degree in Pharmacy
* Registered with the General Pharmaceutical Council (GPhC) and completed pre‑registration training
* Post‑graduate Diploma/MSc in Clinical Pharmacy


Education and Qualifications – Desirable Criteria

* Specialist training, qualification or education in the care of older people and/or frailty
* Non‑medical Prescriber or working towards one
* Member of the Royal Pharmaceutical Society


Previous Experience – Essential Criteria

* Clinical pharmacy practice relevant to acute hospital and either community health services or integrated care settings
* Handling complex medicines information queries/undertaking clinical reviews and providing tailored medication advice/treatment plans for individual patients
* Provision of expert clinical advice including prescribing advice to the wider multi‑disciplinary team
* Writing guidelines, standard operating procedures and policies
* Responsible for policy implementation and development of a service
* Managing, appraising and supervising staff
* Experience of leading on and conducting audits or other practice‑based evaluation
* Delivering training/education sessions


Previous Experience – Desirable Criteria

* Undertaking domiciliary medication reviews
* Experience of practice‑based research and associated publications
* Primary care experience


Skills, Knowledge, Abilities – Essential Criteria

* Understanding of policies and the context around managing long‑term conditions and reducing unnecessary hospital admissions
* Understanding of integrated care systems
* Willingness to challenge practices and systems that may lead to sub‑optimal use of medicines
* Leadership skills including providing leadership in complex circumstances
* Applies sound professional judgement to legal and ethical issues
* Very good clinical skills, including evidence‑based practice, and the ability to communicate highly complex medicines reviews, identify clinical issues and devise treatment plans
* Ability to assimilate, analyse and interpret financial data


Personal Characteristics – Essential Criteria

* Demonstrates ability to meet set targets & manage own time, prioritise workload effectively
* Demonstrates ability and willingness to deputise for senior staff while recognising limitations of experience and ability
* Able to work under pressure & make important decisions in pressured situations
* Self‑motivated

* Understanding of Confidentiality and the Data Protection Act
* Substantial evidence of CPD and continuing education in pharmacotherapy of older people and long‑term conditions
