Overview
candidate will provide rehabilitation and reablement support to individuals following accident, incident or illness allowing a return to independence. This involves following therapy plans and working in conjunction with other health and social care professionals. The role also provides a crisis response to support when unforeseen events occur such as falls and carer breakdown.
Responsibilities
* Direct work such as supporting with moving and handling, attending to personal care needs and administering medication.
* Maximising opportunities for social inclusion and emotional wellbeing by signposting to external organisations and professionals.
* Work within a rolling rota on a 25 hour per week contract; shifts typically between 07:00 and 23:00, e.g., 07:00 to 13:35 or 15:55 to 22:30.
* Opportunity for overtime; weekend enhancements when working weekends.
* As a reablement assistant you will be part of a fast paced and developing service with opportunities for further development and progression.

Requirements / Qualifications
* A previous qualification in care is desirable, though not essential, as full induction/training will be provided.
