Skin/Colorectal Oncology Clinical Nurse Specialist
This is an exciting opportunity to work within the Skin and Colorectal Clinical Nurse Specialist teams. Under direction from the Lead Skin Cancer and Colorectal Nurse Specialist you will assess, plan, implement and evaluate expert nursing care and provide support to Skin and colorectal cancer patients and their carers. The post holder will have continuing responsibility for the co‑ordination and organisation of individual treatment programmes throughout the illness trajectory and work to develop communication across divisional and organisational boundaries.
Provide specialist nursing advice and psychological support to patients, carers and healthcare professionals.
Acts as a resource and contributes to the education and training of other healthcare professionals as required.
Contribute to service development, audit and research projects and help develop educational packages as needed.
Please note this is a fixed term position for 12 months with the possibility of becoming a permanent position.
Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.
Main duties of the job

Job responsibilities
* Manage own caseload of patients while working as part of the multidisciplinary team.
* Communicate highly sensitive and emotive condition related information to patients and relatives (e.g. terminal illness, bereavement).
* Assess new patients in the clinical area, identifying their psychological, physiological, emotional, spiritual, and cultural needs, and provide a pathway of nursing care.
* Undertake follow‑up clinics under supervision.
* If required, initiate drug therapy/medication within agreed clinical guidelines and legal framework.
* Act as patient advocate, educate on treatment options to enable informed choice, and negotiate with the medical team to agree a care pathway.
* Provide continuity of care throughout the patient journey and liaise across specialised areas.
* Contribute to the development of specialised programmes of care and provide specialised advice, written and oral information to patients and relatives.
* Provide clinical advice and support to healthcare professionals within and across organisational boundaries.
* Formulate individualised patient treatment care programmes with the medical/nursing team, considering investigations and organising treatments as needed.
* Act as a role model.
* Provide support for distressed patients, carers and bereaved relatives; identify patients at risk and refer to appropriate agencies.
* Initiate changes in management of care.
* Participate in case conferences regarding future management or discharge arrangements.
* Maintain accurate patient records and ensure all relevant information is documented in medical and nursing notes.
* Collaborate with multidisciplinary team members to assess needs of patients, families and carers, and provide specialist advice.
* Assist with the development of a specialised nursing service provision.
* Prepare patients for procedures and gather feedback on service delivery standards.

Person Specification
Knowledge & Experience
* Demonstrable (significant) and relevant post registration experience, at Band 5 or above, in cancer, skin, colorectal, palliative care.
* Demonstrable experience of teaching.
* Demonstrable experience of audit and research leadership and management.
Qualifications
* NMC Registered Nurse.
* Post‑basic qualification in cancer/palliative care or equivalent relevant subject or experience, or working towards.
* Research qualification.
* Good communication skills, written and verbal, with ability to demonstrate fluency, clarity and effectiveness at all levels.
* Organisation and negotiation skills.
* Leadership and motivational skills.

Salary
£39,959 to £48,117 a year Per Annum Pro Rata
