Responsibilities
Undertake a wide range of delegated clinical tasks in line with Level 4 competency requirements, ensuring safe and effective patient care. Support the assessment, planning, implementation and evaluation of care under the supervision of a Registered Nurse. Contribute to improving the patient experience by promoting independence and supporting patients to maximise their functional ability. Carry out clinical observations, escalates concerns appropriately and accurately document all findings in line with Health Board policy. Assist with personal care needs including washing, dressing, continence support, mobility, nutrition and hydration, maintaining dignity at all times.
Contract & Working Hours
The rota includes a variety of daytime shift patterns, operating Monday to Sunday with no night shifts required.
Qualifications
* Level 4 vocational qualification in a relevant health‑care associated subject or Certificate in Higher Education Health Care Support Worker
* GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications
* Evidence of continuing personal development relevant to the role in health care
* Understanding of the HCSW Code of Conduct, All Wales Delegation guidelines and all Wales Band 4 Assistant Practitioner Governance Framework
* Clinical knowledge relevant to the role
* Demonstrated ability to follow procedures and treatment plans
* Understanding of safeguarding legislation (e.g. Adults/Children, The Mental Capacity Act and Deprivation of Liberties Safeguards)
* Local knowledge of statutory and voluntary services
* Teaching or assessment qualification
* Welsh language: Level 1 speaker desirable (English and/or Welsh speakers are welcome)
Experience
* Significant and recent experience of providing relevant clinical care
* Interpersonal and communication skills; ability to manage/diffuse challenging or threatening situations
* Experience of undertaking a range of clinical skills in accordance with role scope and national governance framework, e.g. catheter care (including removal), monitoring and recording vital observations, reviewing risk assessments, monitoring and interpreting blood glucose, wound care support, palliative care
* Administration of medicines via oral, topical, SC or IM routes
* Skills in non‑complex wound care
* Experience of providing care in a community or primary care setting (for community roles)
