We are recruiting for a Neonatal Outreach Nurse who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.
The goal of the service is to facilitate effective discharge of infants from the Neonatal Unit who require the input of the Neonatal Outreach Service and to ensure infants requiring continuing medical and nursing care at home have a smooth transition from the Neonatal Unit to their home environment.
The post holder is responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care without supervision – undertaking care in the most appropriate environment using a high level of technical expertise.
You participate in internal rotation duties, providing 24 hour nursing cover including neonatal outreach and Transitional care.

Main duties of the job
The post holder will participate as a member of the neonatal multi-disciplinary team providing evidence based family centred care. They must be skilled in communicating effectively with parents, families and members of the multidisciplinary team. They will strengthen the links between the neonatal and paediatric community service and will support nurse led clinics for the administration of vaccines for high risk infants.
The post holder will keep accurate and complete records of activities and communications consistent with legislation, policies and procedures.
The post holder will work as part of the multi-professional team supporting discharges of babies with ongoing special needs both in hospital and as a member of the neonatal outreach team.
To be conversant with and adhere to the agreed policies and procedures of the Trust and Neonatal Services’ Department ensuring care reflects the principles stated in our policy and objectives.
Demonstrate proficiency in assessing, planning, implementing and evaluating neonatal care consistent with evidence based practice for babies and their families in special care and transitional care environment.
Report to the Senior Nurse/Doctor any deviation from the normal, and initiate emergency treatment whilst awaiting medical aid.
To maintain confidentiality surrounding the babies and their families in accordance with the NMC, data protection act and Trust policies.
To implement appropriate individualised discharge care planning liaising with the families, multidisciplinary team and other agencies.
To be responsible and maintain a safe standard of storage and administration of drugs in accordance with the Trust and NMC policies.
Promote an environment conducive to fostering good parent/infant relationship. Promote family centred care.
Keep parents informed and provide support and information to enable them to make informed decisions.
Facilitate counselling of parents and ensure that account is taken of their customs, values and spiritual beliefs.
Work in partnership with parents supporting them in meeting their babies’ ongoing needs at home.

* RSCN/ Degree/Diploma in Child Health or RN Adult Nursing.
* Evidence of continuing professional development
Previous Experience
