Overview
We have an excellent opportunity for a highly motivated individual to join our established Older Adult Community Mental Health Team. This role requires excellent communication and interpersonal skills and adaptability, as no two days are the same. As an assistant practitioner, you will conduct memory assessments at our office base, within care homes and in people’s homes within the locality, and subsequently progress these assessments through the process of diagnosis and treatment pathways where suitable. You will monitor the titrations of anti-dementia medicines and determine when to escalate concerns. Annual monitoring of these medications will also be completed, via telephone and face to face. Reviews on behavioural and psychological symptoms of dementia will also be a big part of your day-to-day role.

Responsibilities
* Conduct memory assessments at base, within people’s homes, and within care homes in the Grantham/Sleaford locality, and complete relevant documentation.
* Monitor titrations of anti-dementia medications and complete annual reviews; document findings accordingly.
* Provide high standard of care and comprehensive assessments for individuals experiencing mental health difficulties, in line with the Trust’s values.
* Support individuals to develop independent living skills and live well at home; assist in accessing the right services for their needs.
* Review people struggling with their mental health, both functionally and with a diagnosis of dementia.
* Manage a caseload and contribute to the Trust’s vision of keeping people well at home within the community.
* Work collaboratively with multidisciplinary team members to ensure effective and appropriate clinical decision-making within the CMHT and with external agencies.
* Assess carers and families’ needs and develop, implement and review programmes of support for carers and families.
* Liaise with stakeholders, including GP surgeries, carers and families, and other teams within LPFT and Adult Social Care; participate in regular joint working with Home Treatment Teams and inpatient wards as required.
Qualifications and Requirements
* Full UK driving licence is required.
