Job Overview
We are recruiting X2 full-time Band 6 Hospital at Home Practitioners to join our Brent Unplanned Care service, based at Sudbury and Wembley. This role delivers acute, hospital-level care directly in patients’ homes.
Key Responsibilities
The Hospital at Home service provides short-term, intensive care for adults with acute conditions who would otherwise require admission to hospital. You will work within a multidisciplinary team of specially trained registered professionals, including GPs, supporting earlier discharge, preventing avoidable hospital admissions and ensuring patients receive the right care, in the right place, at the right time. You will have access to home‑based monitoring equipment, point‑of‑care testing, and deliver a range of clinical interventions including intravenous therapy. You will practise autonomously while being supported by an experienced team.
* Work on a rotational shift basis within the Harrow Hospital at Home service, operating between 8am and 8pm, seven days a week.
* Effectively prioritise and manage your own workload, working with a high level of autonomy and minimal supervision.
