<p>We are looking to recruit an experienced Practice Nurse to join our friendly team. We are a 4 GP partner practice with a practice list size of 11,000 patients.</p><p>We are a high performing QOF/ LES practice and are rated GOOD by CQC . We are looking for a nurse who is passionate about giving the best patient care and helping a wide diversity of patients.</p><p>The required working days will be Tuesday Wednesday Friday.</p><h3>Main duties of the job</h3><p>Main Duties of the Job</p><ul><li>Chronic Disease Management</li><li>Childhood Vaccinations and Immunisations</li><li>Adult Vaccinations and Immunisations</li><li>Cervical Screening</li></ul><h3>About us</h3><p>We have 4 GP partners, 1 Salaried GP, 1 Advanced Nurse Practitioner, a Physician Associate, 3 Practice Nurses and 2 Health Care Assistants who work well alongside our reception and admin team.</p><h3>Job responsibilities</h3><p>PRINCIPLEFUNCTIONS:</p><p>The post holder will:</p><ul><li>Participatein the delivery of high standard of nursing/clinical care.</li><li>Workand liaise with Primary Health Care Team members.</li><li>Beproficient in the management of chronic disease and implement treatmentplans for patients to manage long term conditions.</li><li>Identifyand manage patients at risk of developing long term conditions</li><li>Advisewomen on contraception and sexual health.</li><li>Managecervical cytology consultations, obtaining cytology samples in accordancewith the cervical screening programme and competencies.</li><li>Administervaccinations and Immunisations for Adults and children following nationalprogrammes.</li><li>Advise,support and administer vaccinations for patients travelling abroad.</li></ul><p>The provision of nursing treatment and assessment of patientsin participation with the GPs or independently according to agreed anddocumented protocols.</p><p>Carry out the following when required:</p><ul><li>Asthma& COPD Reviews</li><li>DiabeticReviews</li><li>DementiaReviews</li><li>Venepuncture</li><li>Childhoodimmunisation and vaccinations</li><li>Traveland routine vaccinations</li><li>Administerprescribed therapies i.e. Vit B12, Depo Provera, neuroleptic medicationetc recognising side effects andcontraindications</li><li>Electrocardiography</li><li>CHDreviews</li><li>Minorsurgery support including Vasectomy</li><li>Requestpathology tests eg urine culture, swabs etc</li><li>Responsiblefor infection control and clinically based protocols includingimplementation and training across the practice.</li><li>Firstaid and management of an Emergency</li><li>Orderand maintain treatment room stock and vaccinations, ensuring good rotationand adequate stock levels</li><li>Undertakehome visits if required.</li><li>Attendand participate in practice/Nurse meetings, as appropriate.</li><li>Participatein quality assurance processes such as clinical audit, evaluation andserious incident recording, ensuring any lessons learned are translatedinto practice.</li><li>Analysedata and develop initiatives to improve the uptake of national screeningand immunisation programmes including coordinating new vaccinationcampaigns.</li><li>Whererelevant for the nursing team, ensure correspondence and safety alerts areactioned.</li><li>Maintainand enforce standards of record keeping and associated administration inaccordance with organisational policy and local joint arrangements andprofessional guidelines.</li><li>Recogniseopportunities to reduce inequalities and unwarranted variation in healthcare for the practice population, as outlined in the nursing frameworkLeading Change-Adding Value (May 2016)</li></ul><p>Advise on the promotion of health and the prevention ofillness, through the provision of health education and lifestyle counsellingrecording such information as height, weight, smoking status, and alcoholstatus onto the EMIS records.</p><p>Order promotional health related material</p><p>ADMINISTRATION</p><p>The post holder must revalidate to maintain theirregistration with the NMC and maintain their Continued Professional Developmentportfolio.</p><p>Maintain clinical and professional knowledge by identifyingtraining needs at appraisal and attending training and updates in accordancewith individual and Practice requirements.</p><p>Liaise with members of the Practices Multi-DisciplinaryTeam.</p><p>Liaise with the Practice Manager.</p><p>It is the responsibility of each individual employee to beaware of the principles of good health and safety and Infection Control withintheir own workplace and to have regard for any directions concerning Health andSafety and Infection Control within their place of respond accordingly, using all available tools eg Languageline, Sign etc.</p><h3>Person Specification</h3><h3>Qualifications</h3><ul><li>A certified Registered Nurse</li><li>Have a current NMC registration</li><li>Be registered with the relevant professional body</li><li>Be a specialised practitioner qualified in Practice Nursing</li><li>Have clinical skills (cytology, immunisation, vaccination, minor surgery)</li><li>Have knowledge of patients with long term conditions</li></ul><h3>Disclosure and Barring Service Check</h3><p>This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.</p>
