Assistant/Trainee Assistant Practitioner

We are looking for an Assistant Practitioner or a candidate who is prepared to work towards the Assistant Practitioner qualification. The successful applicant will work within their own delegated caseload, making day‑to‑day assessment of patients’ conditions and liaising closely with the ICC MDT and other agencies regarding treatment progression, patient safety and ongoing care.
Key Responsibilities
* Provide support and assistance to registered clinicians in the delivery of community rehabilitation and nursing services.
* Assist in the assessment and management of patients, respecting privacy, dignity, individuality and promoting independence and wellbeing.
* Maintain a delegated caseload of patients identified by an allied health professional or registered nurse, prioritising workload appropriately.
* Use specialist skills and knowledge to offer flexible care for patients with physical, psychological, social and environmental needs to enable independence and well‑being.
* Implement interventions that enable patients to achieve personal goals, using continuous assessment and adjusting care to meet fluctuating needs; evaluate effectiveness.
Training & Development
The Trust will provide a 12‑month preceptorship period for new-to-registration Nursing Associates. Returning to practice members may access an agreed preceptorship period.
