An excellent opportunity has arisen for a Rehabilitation Assistant to support a young adult male with complex needs within both a care home setting and the community. This is a rewarding and hands-on role, working closely with a wider care team and therapists to deliver structured rehabilitation support and day-to-day care.
The successful candidate will play a key role in supporting therapy programmes, daily activities, and social engagement, helping to promote independence, wellbeing, and quality of life.
What we need from you:
A minimum of 2 years previous experience working with therapy programs for individuals with additional or complex support needs
Experience working on a one-to-one basis within a care or support setting
A positive, energetic and flexible approach
Strong communication skills and the ability to work as part of a wider team
A caring, patient and professional manner
Full UK driving licence and willingness to drive the clients vehicle
Ability to follow care plans and therapy programmes set by healthcare professionals
Flexibility with working hours, including weekends and occasional overnight support
Desirable:
Experience administering medication within a care or support setting
You must be able to confidently perform the following tasks:
Support the client with all aspects of daily living, including personal care, mobility, feeding and communication
Assist with structured rehabilitation programmes and therapy activities, following guidance from therapists
Encourage participation in social, leisure and community-based activities
Work closely with the wider care team, family members and healthcare professionals
Support the client to maintain independence and achieve rehabilitation goals
Accompany the client to appointments and activities as required
Maintain accurate records and provide feedback on progress
Ensure the clients wellbeing, safety and dignity are maintained at all times
Assist with household tasks and maintaining a safe environment where required
Salary: Weekdays £25 per hour/Weekend days £27 per hour/Sleep-in rate £85.00 per shift (10pm 7am).
Hours: 30 hours per week, with flexibility required across weekdays, weekends and occasional overnight or holiday support, working on a rota.
