Highly Specialist Stroke Speech & Language Therapist

We have an exciting opportunity for motivated and dynamic Speech and Language therapist to join our South Nottinghamshire Community Stroke Team at Nottinghamshire Healthcare NHS Trust. The role is part time 0.8wte.

You will be joining a forward thinking, innovative, caring and person centred integrated team that are able to treat clients up to 12 months post stroke. The team has a focus on compassionate leadership, staff well-being and valuing every member of the team for their unique contributions.

Post-graduate dysphagia training is essential for this post, and you will be supported to continue to develop your specialist skills with training opportunities, supervision, peer support and developmental days.To see the full detailed job description and main responsibilities of the job please refer to the job description.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.


Main duties of the job

This role requires day-to-day delivery of care in our Integrated Community Stroke Multidisciplinary Team of AHP's, together we strive to make the difference to the community we serve.

Main duties would be:

o To demonstrate your passion, clinical and professional expertise in order that those with speech, language and communication needs and/or dysphagia needs benefit from a well-coordinated pro-active, specialist, efficient and effective speech and language therapy service.

o Support our clients to enable them to live their optimum life, being self-caring and independent with their health needs following a stroke.

o Work collaboratively within a multidisciplinary framework, providing compassionate, realistic and timely support to a diverse client group and their families.

o Demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful and approachable, evidencing high levels of personal and clinical credibility, being able to establish effective working relationships in a range of work settings.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Job responsibilities

We are looking for a Highly Specialist Speech and Language Therapist to join our dedicated Community Stroke Team across the South of Nottinghamshire.

This post will allow the successful applicant the opportunity to work closely within an integrated multi-disciplinary team, providing therapy to clients referred to the Community Stroke Rehabilitation services, including early support stroke rehabilitation. The multidisciplinary team includes Occupational Therapy, Speech and Language Therapy, Physiotherapy, Clinical Neuro-Psychology, Assistant Practitioners and Rehabilitation support workers. Reviewing officers provide a post stroke review for clients at 6 and 12 months.

As a Community Stroke service, we aim to offer a high quality therapy led Multidisciplinary Team approach providing stroke rehabilitation in a patients home or care home, focused around achievement of agreed rehabilitation goals.

As a Highly Specialist Speech and Language therapist in the Community Stroke team you will be expected to:

- Manage a caseload using highly specialist Speech and Language Therapy assessment for stroke clients who may have complex and diverse presentations, and plan appropriate management and treatment using specialist advanced clinical reasoning skills and a wide range of therapeutic skills- To provide high quality holistic evidence-based care to patients/service users that is responsive, effective and efficient.- To facilitate and support timely discharge and prevent inappropriate admissions by supporting patients / service users to remain as independent as possible and maintain or improve their level of function and quality of life.- To assist in the development and implementation of appropriate pathways of care for the Stroke pathway- To provide locality team leadership and deputise for the Team Lead when needed and provide support with the clinical development and management of staff.- To be pro-active in delivering responsive and anticipatory care to the wider community, promoting independent healthy lifestyles, reducing health inequalities, and tackling underlying determinations of health, to improve overall qualities of life in the local population.- To provide some operational leadership; ensuring commissioned targets & key performance indicators are met.- To provide co-ordination, support and clinical supervision of Speech and Language Therapists, assistants, and students- To support the organisation and delivery of in-service training and initiate informal training sessions when required to ensure high standards of clinical care.- To keep up to date with innovation in AAC equipment and App development.- To facilitate diverse and high-quality therapy placements across the service, support staff to develop and enjoy positive roles and experiences as therapy clinical educators which contributes to key AHP workforce strategies.

The Community Stroke Team is split into three localities. The base of this role is within in the locality of Nottingham North and East. The team work closely and will require you to work flexibly, across the whole of South Nottinghamshire, to support our evolving and developing service, supporting the team to deliver high quality integrated care.

This position requires you to work independently, but also be a team player, working closely with our Integrated Care Provider partners providing care to a diverse population covering many different geographies including, Broxtowe, Gedling and Rushcliffe.

Qualifications

* Recognised Speech & Language Therapy Degree Qualification.
* Registered member of RCSLT
* HCPC Registered

Knoweldge

Experience

* Experience of working with specialist client group Stroke/Neuro
* Experience of working in the community or outpatient setting
* Experience of designing and delivering different learning and development opportunities for staff teams
* Experience of delivering environmental approaches to SLT service delivery and evaluating them
* Experience in developing evaluation / audit tools and resources as part of SLT team
* Advanced dysphagia skills
