SCPHN School Nurse

DCHS has an opportunity for a SCPHN School Nurse to join the Children's Community Services Team. There are 2 positions (30 & 24 hours per week).

You will need to effectively engage with your client group, peers and partner agencies in all elements of your role, leading by example and promoting a supportive and caring culture.

This will require an inclusive, transparent, open approach to your role, where you are happy to be accountable to others and to go that extra mile when required.

You will be passionate about our business, creating a culture of continuous improvement, where patient welfare is at the heart of all you do.

Working with the skill mix team, you will deliver the Healthy Child Programme universally along with progressive levels of service and safeguarding children according to identified need.

You will provide services in partnership with the local voluntary sector and multi‑agency teams and participate in service improvement both within the locality and across the county.

Main duties of the job

You will manage and develop the Children's Community Services Team (5‑19) and contribute to the development of services to deliver the commissioned service specification.

You will be part of the multi‑disciplinary team including the wider local children's workforce, working within a public health framework, to meet the prioritised health needs of children and families.

You will have responsibility for delivery of the key performance indicators for a community of school aged children within an identified specific action plan.

You will undertake responsibility as 'named nurse' for a Multi‑Agency Team area and inclusive schools within a specified locality.

You will carry continuing responsibility for the assessment of health needs and the development, implementation and evaluation of programmes of intervention. To create and promote a positive learning environment for junior staff and students on placement.

Whilst in this post, you will have an office base at Brimington Clinic and cover schools in the Chesterfield area.

Traveling to other parts of Derbyshire is sometimes necessary, for example to attend meetings and training sessions or provide service cover.

Person Specification


Qualifications

* NMC first level registered nurse
* Evidence of recent post registration learning
* Nurse Prescribing
* Recognised teaching and assessing qualification


Experience and Knowledge

* Sound understanding and awareness of community and public health agenda
* Developing and devising public health initiatives
* Teaching experience


Skills and Attributes

* Clinical Leadership
* Basic IT skills
* Requirement to travel across the geographical area/driver
* Experience of working with electronic patient records, along with knowledge of associated record keeping policies and Caldicott principles
* Knowledge of using social media
