What the Job Involves Your primary role will be within the Adult Services Review Team based at County Hall, where you will closely collaborate with the Team Manager, Social Workers, and Duty Workers. Key responsibilities include organizing and monitoring services in accordance with individual care plans, providing advice and practical assistance, liaising with voluntary and statutory agencies, and conducting annual care plan reviews or reassessments. You will participate in team meetings, carry out duties in line with Council Policies, and contribute to equality of opportunity initiatives. The role might require you to work from various locations and to actively ensure workplace health and safety.
