We have an exciting opportunity for an experienced Nurse to joinour team at Sothall practice, on a permanent 27 hours per weekcontract.

Main duties of the job

We are looking for a reliable, enthusiastic and motivated PracticeNurse to work with us. A nurse who can provide excellent care and has a truededication for improving health outcomes for patients.

The successful candidates will be a registered nurse, ideally withexperience of working in General Practice.

The post holder will work as a member of the Practice Nursing Teamand be responsible for acting within their professional boundaries to providecare for the presenting patient including screening, clinical assessment,diagnosis, treatment and follow up of care; demonstrating safe clinicaldecision making and expert patient care.

They will be involved in all areas of Chronic Disease Management(Asthma, Diabetes, COPD etc.)

The work is interesting, often challenging but hugely rewarding.The team is very mutually supportive, with excellent experienced clinical,management and administrative support on site.

On appointment you will have a full induction into the service andsupport will always be available when needed from our Clinical Nurse Lead.

Primary Care Sheffield (PCS) is a GP-led organisation. Ourshareholders are 75 GP practices in Sheffield covering over half a millionpatients. As well as providing a unified voice for general practice in thecity, we play an active role both within our Accountable Care Partnership (ACP)and the wider South Yorkshire and Bassetlaw Integrated Care System (ICS).


Job responsibilities

1. Job Summary

Towork as a member of the Practice Nursing Team and be responsible for actingwithin their professional boundaries to provide care for the presenting patientincluding screening, clinical assessment, diagnosis, treatment and follow up ofcare; demonstrating safe clinical decisionmaking and expert patient care.

Tobe involved in all areas of Chronic Disease Management (Asthma, Diabetes, COPDetc.)

The Practice Nurse will work collaboratively with theclinical team which will include GPs and other health care professionals.

2. Main Duties andResponsibilities

Key Result Areas

2.1 Clinical Care

* Assist in and performroutine tasks relating to patient care
* Undertake and promotecervical cytology
* Provide a comprehensivefamily planning service and sexual health service
* Undertake Spirometryand have undertaken relevant training
* Manage newly diagnosedpatients with Asthma, COPD & Diabetes
* UndertakeChronic disease management and prevention Asthma, COPD, Diabetes, Hypertension,Hypothyroidism, IHD, Stroke & LD
* Perform wound careusing aseptic technique and general wound care
* Undertake opportunisticscreening of BP checks, height, weight, BMI, urinalysis, glucose monitoring andsmoking status.
* To undertake phlebotomy
* Administer injectionsunder an individual prescription or patient group direction
* Offer a holisticapproach to travel health including comprehensive advice for patients prior to travel.
* Undertake ear syringing.
* Support patients andcolleagues in adopting sound infection control measures
* Competent inanaphylaxis and resuscitation techniques
* Child Health and ChildProtection
* Promote health andwell-being through the use of health promotion, health education, screening andtherapeutic communication skills
* Refer and use socialprescribing teams
* To undertake homevisits
* To clearly communicatethe health status of the patient using appropriate terminology, technology andformat.
* INR monitoring ifagreed by a shared care protocol.
* To supervise and assistjunior members of the nursing team, NAs, trainee NAs, HCAs.
* Undertake infectioncontrol measures, including audits and annual reports.

2.2 Professional

* Adhereto the Nursing and Midwifery Council (NMC) Code of Professional Conductand Scope of Professional Practice.
* Ensure that professionalpractice adheres to organisational policies, procedures and guidelines
* To promote and maintainown personal development and clinical excellence.
* Participate inorganisational decision making, interpret variations in outcomes and use datafrom information systems to improve practice.
* To promoteevidence-based practice through the use of the latest research-based guidelinesand the development of practice-based research.
* Monitor theeffectiveness of their clinical practice through the quality assurancestrategies such as the use of audit and peer review
* Participate incontinuing professional development opportunities to ensure that up-to-dateevidence-based knowledge and competence in all aspects of the role ismaintained
* To regularlyparticipate in clinical supervision as agreed with Lead Nurse.

2.3 Communication

* Communicate effectivelywith patients and families regarding information about their care
* Provide advice andinformation to patient, carers and their families where appropriate
* Communicate effectivelyand work collaboratively with medical, nursing and allied health professionalsto ensure delivery of a co-ordinated service
* Ensure accuratedocumentation record keeping in line with local policies and NMC guidelines
* Promote and demonstrateeffective communication networks within the organisation, with partnerorganisations and other service providers

2.4 Organisational

* To complete records,audits, reports and respond to appropriate questions and requests.
* To help developprotocols and assist in audits and research.
* To attend meetings asrequested.
* Support effectivecommunication channels between the whole team.
* To work according toClinical Governance and support the Clinical Governance Agenda.
* To understand the roleof Risk Management within the Practice setting and contributing to riskassessment and Significant Event Audit and the importance of Infection Control.

2.5Educational

* To contribute to the development and maintenance of an excellentlearning environment
* Participate in defining own developmental plan to maintain and developown clinical skills and ensure own professional development
* Undertake and complete all statutory and mandatory training
* Contribute to their own personal development and participate in anappraisal and regular performance reviews with the Lead Nurse
* To take part in clinical supervision within the organisation
* Attend any training and keep skills and knowledge up to date, inlinewith NMC CPD requirements

2.6 Clinical Governance

* To be familiar with andcomply with the Practices Clinical Governance standards, policies andprocedures.
* Undertake any SEAs thatyou have been involved in.
* Work with the Lead Nurseto investigate adverse clinical incidents and complaints

2.7 Other

* Demonstrate commitment to flexible working patterns and across sites tomeet the needs of the service and staff
* To maintain patients confidentiality at all times
* This post is deemed to require a Disclosure check

General

The main duties andresponsibilities shown above are not exhaustive but should merely be regardedas a guide. The post-holder will be expectedto conduct any reasonable activities according to the service needs at thattime. These will be subject to periodicreview and may be amended to meet the challenging needs of the service. Thepost-holder will be expected to participate in this process and theorganisation would aim to reach agreement to changes.

Infection Control

Staff are responsible for protecting themselves and others againstinfection risks. All staff regardless ofwhether clinical or not are expected to comply with current infection controlpolicies and procedures and to report any actual or potential infectionprevention and control risks to their managers immediately

Confidentiality

Allstaff and contractors working for the organisation have both a common law dutyand a statutory duty of confidentiality to protect patient (and indeed anypersonally identifiable) information and only use it for the purposes for whichit was intended. The disclosure and use of confidential patient information needs to beboth lawful and ethical.

Information Governance

All staff must keep up-to-date with the requirements of Information

Governance and must follow organisational policiesand procedures to ensure that the organisation information is dealt withlegally, securely, efficiently and effectively. Staff must appropriately manageall the Information they handle during the course of their employment, makingthe information available for sharing in a controlled manner, subject tostatutory requirements.

Records Management

All employees have a legal responsibility for allrecords worked with e.g. patient records, financial records, personal,administrative etc., that are gathered or used as part of working within theorganisation. The records may be held in a variety of formats such as paper,electronic, microfiche, audio and video tapes etc. Any doubts regarding the correct managementof records must be discussed with immediate line manager.


Person Specification


Knowledge

* Understanding and knowledge of policy developments related to the delivery of primary care services within a GP Practice
* Understanding of evidence based practice
* Knowledge of national standards that inform practice (e.g. National Service Frameworks, NICE guidelines etc)
* Understanding of equal opportunity and diversity issues
* Understanding and experience of interagency working and adult/child safeguarding.
* Ability to follow and undertake policy and procedures


Skills

* Ability to assess and manage patient risk effectively and safely
* Well-developed word processing/data collection/IT skills
* Excellent interpersonal, verbal and written communication skills
* Time management and ability to prioritise workload
* Ability to work under pressure
* Able to analyse data and information, drawing out implications for the individual patient/impact on care plan
* Able to establish and maintain effective communication pathways within Primary Care and with key external stakeholders
* Clean UK Driving Licence Essential


Experience

* Experience:
* Minimum of 2 years post registration experience
* Experience of working in Primary Care
* Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence in all aspects of the Practice Nurse Role
* Experience in the management of long term conditions e.g. asthma, COPD, diabetes, CHD, Health Promotion/Education
* Vaccine and Immunisation training
* Cervical Cytology
* Evidence of working autonomously and as part of a team
* Proven ability to evaluate the safety and effectiveness of their own clinical practice
* Undertake Home visits
* Willingness to undertake further development opportunities.
* Experience of working with vulnerable patients
* Experience of working in areas of deprivation


Qualifications

* Registered General Nurse (Currently registered with the Nursing & Midwifery Council, with a valid pin and no restrictions)
* Evidence of further study/professional development.
