Responsibilities
* Provide planned community nursing care to patients in the South Shrewsbury locality, supporting timely discharge and enabling patients to realise their potential.
* Work within an interdisciplinary team and promote innovative delivery of care.
* Cover a 7‑day service (8am to 6pm).
* Participate in a preceptorship programme and pursue ongoing professional development in a nurturing environment.
Qualifications
* RGN
* NMC Registered
* Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
Experience
* Post-registration general experience
* Experience of working in a multi-disciplinary / multi-agency health/social care environment
* Experience and knowledge of basic administration systems including Microsoft Office
* Experience of successful partnership working with other services
* Previous community experience
Knowledge and Skills
* IT skills or willingness to undertake training (e.g., RIO)
* Up-to-date Mandatory Training
* Uses evidence-based practice
* Works within defined competencies
* Aware of changing healthcare factors influencing change; tact and diplomacy
* Strong verbal and written communication; ability to gain rapport at all levels
* Engaging and empowering approach to work with others and deliver outcomes

* Must be able to meet the transport requirements of the post
