Our client is proud to be recruiting for a Service Coordinator to join their dedicated team. As a Service Coordinator, you will be working to deliver high-quality support. You’ll support customers who have complex needs and who are experiencing multiple disadvantages, including substance use, mental health, offending, and rough sleeping/homelessness. Reporting to the Support Team Leader, you will coordinate customer access to a wide range of services, and you will lead the design and delivery of a service coordination plan alongside the customer and other essential agencies.
Job requirements
* Provide flexible, compassionate support to customers with complex needs, including homelessness, substance use, mental health.
* Empower customers to regain independence and help them understand what it takes to manage their own tenancy.
* Work closely with the multi-agency team and support network to strengthen support.
* Develop and design tailored service/support plans.
* Keep clear, accurate records and stay up to date on housing and safeguarding laws.
* Organise and chair team meetings to provide an effective platform for communication.
* Work alongside the Support Team Leader to promote their services, build community relationships and monitor service impact.
What they’re looking for
* Experience supporting adults with complex needs such as substance misuse, homelessness and ill mental health, or transferrable skills you can bring to the role.
* Experience working with confidential and complex information, following data protection laws.
* Genuine compassion for people affected by homelessness and an understanding of the barriers they face.
* Have a basic understanding of all Microsoft applications including Outlook, Word, Excel and Access.
* Advocacy skills and confidence to challenge decisions.
* Great communication and problem-solving skills.
* Ability to adapt and creatively overcome barriers.
* Strong organisation and time management skills.
* Be able to prioritise high-risk cases.
* Full UK driving licence with own vehicle for work purposes.
* DBS check.
