Job Title: Reablement Support Worker
Location: Wigton and Solway
Salary: £13.47 per hour
Hours: 1 x 30 hours and 1 x 24 hours
Contract: Fixed Term (6 Months)
About this opportunity
Reablement Support Workers will be responsible for working with people identified with reablement needs to ensure key accountabilities are met.
Driving licence is essential.
Job Accountabilities:
* Reablement goals/outcomes are met in accordance with the reablement plan.
* Any additional goals/outcomes that may improve quality of life and promote independence are identified during the reablement episode.
* Feedback about additional identified goals/outcomes is fed back to the Reablement and Assessment Officer.
* Clear and timely communication with the Reablement and Assessment Officer regarding the individual's identified goals/outcomes being met.
* Close team working within a small dedicated team of reablement support workers, communicating effectively about progress or concerns in achieving goals.
