Qualified Social Workers required within Physical Disabilities team
3-6 Month Contract
Rate £38 Per hour (Umbrella Rate)
Location: Hampshire – hybrid working (1 day a week in the office / face to face visits). We will try to match you with the nearest office to where you live.
Responsibilities
1. Undertake Section 9 Care Act 2014 assessments, identify eligible needs, and determine appropriate care and support provisions.
2. Complete unplanned support plan reviews, ensuring responsive reassessment and timely adjustments to care packages in line with changes in need or risk.
3. Risk assessment and safeguarding, applying legislative frameworks effectively across both mental health and physical disability cases.
4. Support adults with mental health needs, with working knowledge of the Care Programme Approach (CPA) and the interface between the Mental Health Act and the Care Act.
5. Understand physical disability services, including long-term conditions, mobility and access needs, rehabilitation, and technology-enabled care solutions.
6. Manage complex, dual-diagnosis cases involving coexisting mental and physical health conditions, coordinating across agencies and specialisms.
7. Experience in Multi-Agency Safeguarding Hub (MASH) involvement: screening, prioritising, and triaging safeguarding referrals; multi-agency working and thresholds for adult safeguarding; time-sensitive decision making.
8. Adopt a strengths-based and person-centred approach, collaborating with individuals, families, and communities to promote independence and minimize reliance on formal care; develop plans that avoid restrictive or institutional responses where possible; use creative problem-solving for cost-effective, least restrictive outcomes.
Qualifications and Experience
Skills and Experience Required
* Statutory Assessment and Planning Expertise: experience in undertaking Section 9 Care Act 2014 assessments, identifying eligible needs, and determining appropriate care and support provisions; skilled in reviews and timely adjustments to care packages.
* Multi-Agency Working: strong ability to coordinate across agencies, safeguarding knowledge, and robust risk assessment.
* Knowledge Across Client Groups: practical experience supporting adults with mental health needs; working knowledge of CPA and interface between Mental Health Act and Care Act; understanding of physical disability services, long-term conditions, mobility and access needs, rehabilitation, and technology-enabled care solutions; ability to manage complex, dual-diagnosis cases.
* MASH Involvement: proven experience working within or alongside a MASH with high-level screening, prioritising, triaging safeguarding referrals, and evidence-based decision-making in time-sensitive situations.
