We offer development and training opportunities and are seeking motivated, passionate Band 5 Community Nurses aiming to develop into:
* Band 6 District Nurse after completing the District Nurse Qualification.
* Chronic Condition Case Manager through disease-specific training.
* Nurse Practitioners via a modular clinical skills development approach.

The successful candidate will have excellent communication skills and the ability to deliver effective clinical care within the District Nurse Team. The Central Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across Conwy and Denbighshire, working with partners to provide high-quality, evidence-based care aimed at maintaining patients in their homes, reducing hospital admissions, empowering self-management, and delivering seamless, patient-centered care.
You must be able to travel to patients’ homes in both urban and rural areas promptly.
This is a rotational 24-hour post.
Welsh language skills are desirable; applications from Welsh and English speakers are equally welcome.
