Community Integrated Care is currently seeking a dedicated and compassionate health and social care professional with Complex Care experience for a Service Leader based in Newcastle upon Tyne.

Responsibilities
* Lead and motivate a team of support workers in a complex care environment.
* Drive high standards of care and service delivery and promote an inclusive and empowering culture.
* Ensure services are well‑resourced, compliant and fully meet CQC registration and quality assurance requirements.
* Manage intensive support planning and care management for individuals with learning difficulties, communication or autism challenges.
* Support the development of staff through training and supervision.
* Operate basic IT systems required for the role.
Qualifications
* Experience working in a complex care health or social care setting.
* Experience supporting people with learning difficulties, communication or autism who require intensive support planning.
* Professional qualification in Health and Social Care (e.g. Nurse, Social Worker, SVQ Level 3 with supervisory units, or Care Management Level 3).
* Basic IT skills.
* Strong interpersonal skills: patience, empathy, compassion, reliability, honesty, determination and resilience.
