Cardiac Surgery Senior Diabetes Nurse Specialist. Position based at WTWA Wythenshawe. Registered Nurse Band 7. 30 hours. Permanent position.

Responsibilities
* Provide diabetes care expertise for cardiac surgery patients, including information, support, advice and specialist knowledge to patients, their families and carers.
* Coordinate patient care to ensure continuity throughout the patient’s care pathway.
* Contribute to review and development of the service to reflect patient needs.
* Review HbA1c in all new patients on the cardiac surgery waiting list and non-elective list; optimise management of existing patients.
* Liaise with inpatient wards, outpatient departments, primary care, Consultant Physicians and GPs as necessary.
* Demonstrate knowledge and experience of diabetes management and clinical leadership skills.
* Monitor and improve standards of care through clinical audit.
* Be willing to undertake further training and development as required to meet post requirements.
* Support delivery of a high-quality, safe and compassionate healthcare service and model the MFT values.
Qualifications & Requirements
* Experience as Clinical Nurse Diabetic Specialist; able to work with cardiac surgery patients.
* Willingness to undertake further training and development as required.
* Ability to liaise effectively with multi-professional teams and primary care.
