Advanced Social Worker Practitioner - Adults Community Team - Barnsley, UK - £37 Hourly
This is an exciting opportunity for a seasoned Advanced Social Worker Practitioner to join the Adults Community Team in vibrant Barnsley for a 3-month locum position. With a competitive hourly rate of £37 and a full-time schedule of 37 hours across Monday to Friday, this role lets you work on-site three days a week with the flexible option of two days from home, perfect for maintaining work-life balance. What you will do:
* Work collaboratively with the team manager to support the locality team.
* Engage with individuals over 18 in the community to complete Care Act Assessments.
* Incorporate the Mental Capacity Act and DOLS while assisting with court work.
* Support with Continuing Health Care Assessments to ensure comprehensive care provision.
* Be proactive in identifying needs and developing care plans in collaboration with other professionals.
