please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses. We are looking for an experienced and motivated Band 7 PCN Lead to join our community nursing team, supporting high‑quality care for housebound patients in their own homes. As a Clinical Nurse Specialist, you will provide strong clinical leadership while overseeing the day‑to‑day management of staff and ensuring the delivery of safe, effective and compassionate care across your teams. In this senior role, you will use your advanced community nursing expertise to offer sound clinical judgement, act as a role model and support colleagues in managing complex caseloads. You will be responsible for triaging new referrals, coordinating care delivery and allocating work to team members with the appropriate skills and competencies. As the senior clinician, you will lead on problem‑solving, decision‑making and planning care for patients with complex needs. You will also take responsibility for workforce planning and rota management, ensuring staffing levels meet service demand. Key Responsibilities
* Provide strong clinical leadership and oversee day‑to‑day management of staff.
* Triage new referrals, coordinate care delivery and allocate work to team members with the appropriate skills and competencies.
* Lead on problem‑solving, decision‑making and patient care planning for complex needs.
* Manage workforce planning and rota management to ensure staffing meets service demand.
* Develop and support workforce, promote wellbeing and ensure the team has the skills, confidence and support needed to deliver safe and effective care.
* Work collaboratively with Primary Care Networks and the wider MDT to coordinate shared care approaches.
* Act as a role model for the profession, actively contributing to service improvement projects with a focus on patient safety, experience and continuous development.
* Support the service operating seven days a week, including bank holidays and weekends, with core shifts between 08:00 and 18:00.
Qualifications & Experience
* Band 7 PCN Lead experience or equivalent clinical leadership role.
* Advanced community nursing expertise and strong clinical judgement.
* Experience with complex disease management and patient safety.
* Experience in workforce planning, rota management and staff development.
* Excellent communication skills and the ability to work collaboratively across teams.
