Hold a clinical caseload, providing comprehensive nursing assessments to determine the appropriate category of care required for a care home placement, and to support the Continuing Health Care and NHS Funded Care process.
Foster robust, open and positive working relationships with all partners, in particular, Local Authority and other mental health services and the 3rd sector, ensuring that all partners are treated with dignity and respect.
Work as part of a multi-disciplinary team, acting as a resource to provide nursing advice in relation to appropriate placement and management.
The post holder will provide comprehensive nursing assessments to determine the appropriate category of care required for a care home placement and to support the Continuing Health Care and NHS Funded Nursing Care process.
The post holder will develop and maintain a collaborative approach to the fair application of both NHS Funded Nursing Care and Continuing Health Care criteria.
The post holder will encourage joint working and actively work towards improving outcomes for patients via the process and provide accurate advice and information to the multi-disciplinary team and families regarding the outcome of the nursing assessment process.
The post holder will frequently work as a Lone Worker.
Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn't mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn't 'fluency ', just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.
