Overview
Are you looking for a new challenge? East of England Community Health and Care NHS Trust is looking for a Registered Band 6 Physiotherapist to join the Inpatient Community Therapy Team based at North Walsham Hospital. The post is at North Walsham Memorial Hospital. You will work as a member of the therapy team on the ward alongside registered Physiotherapists, Occupational Therapists, Assistant Practitioners and Therapy Healthcare Assistants.
Responsibilities
* Deliver high‑quality physiotherapeutic interventions and health care to patients, providing proactive, holistic case management and rehabilitation while working with patients to set realistic goals and using health‑coaching skills to engage patients and their families.
* Assess and prescribe care programmes for patients, including but not limited to those with long‑term conditions, frail and elderly, palliative care and rehabilitation needs, in order to achieve quality of life and independence where possible.
* Assess and prescribe exercise programmes for patients with similar groups, to achieve quality and independence where possible.
* Work within the integrated team to facilitate early discharge from hospital.
* Collaborate with all health care professionals, statutory and non‑statutory agencies to provide a seamless integrated service to our service users.
* Provide advice, support and supervision to junior staff, therapy assistants and students.

Support and Development
You will receive support and development through in‑service training, regular appraisals, a personal development plan and clinical supervision. You must be self‑motivated, able to demonstrate sound clinical reasoning skills and have good communication skills.
Working Environment
Based at North Walsham Hospital Inpatient Unit you will work with a small team of friendly, approachable physiotherapists, occupational therapists, therapy assistant practitioners and registered nurses and healthcare assistants. The 24‑bed inpatient rehabilitation unit provides admission avoidance and supported discharge using an integrated approach to care and rehabilitation through close multidisciplinary working.
