Overview

Halo PCN which makes up Harvey Group Practice and Lodge Health Partnership is recruiting for a Clinical Pharmacist. Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-centred, coordinated care across the district.

The post holder will work within their clinical competencies as part of a multidisciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, manage medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, and acute prescription requests.

The post holder will perform face to face medication reviews for patients with polypharmacy, especially older people, those in residential care homes, and those with multiple comorbidities. They will provide leadership on quality improvement and clinical audit and manage aspects of the Quality and Outcomes Framework.

This role is pivotal to improving the quality of care and operational efficiencies, so it requires motivation and passion to deliver an excellent service within general practice.

The post holder will be supported to develop their role to become a non-medical prescriber, if that qualification is not already held.

20 hours per week, however there may be some flexibility.


Main duties of the job

We are seeking a highly motivated clinician to join our team, delivering high-quality, patient-centred care with a focus on medicines management and long-term condition support. The role involves:

* Running patient-facing clinics, including long-term conditions, medication reviews (practice, care homes, domiciliary), and minor ailment management.
* Providing medicines support, advice, and education to patients and practice staff, both face-to-face and via telephone.
* Supporting safe prescribing practices through repeat prescribing, discharge medicines reconciliation, and implementation of local and national guidelines.
* Contributing to medicines safety, quality improvement initiatives, risk stratification, and reducing unplanned hospital admissions.
* Leading on service development, public health initiatives, and CQC compliance.
* Promoting collaborative working arrangements across the multidisciplinary team.
* Supporting staff development through education and training.

Job responsibilities

Person Specification


Qualifications

* 1. Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
* 2. Demonstrates an understanding of, and conforms to, relevant standards of practice.
* 3. Follows professional and organisational policies/procedures relating to performance management.
* 4. Mandatory registration with the General Pharmaceutical Council.
* 5. Membership Primary Care Pharmacy Association (PCPA).
* May hold or be working towards an independent prescribing qualification.


Experience

* 1. Minimum of two years experience as a pharmacist, demonstrated within a practice portfolio.
* 2. Experience and an awareness of the breadth of common acute and long-term conditions that are likely to be seen in general practice.
* 3. Demonstrates ability to integrate general practice with community and hospital pharmacy teams and community groups.
* 4. Experience of partnership/collaborative working and of building relationships across a variety of organisations.
* 5. Experience of working in primary care.
* 6. Experience of working in a GP practice.
