Job overview

We are seeking a caring, motivated, and adaptable individual to join our Crisis Resolution Home Treatment Team (CRHTT) as a Support Worker. You will have a genuine interest in mental health, excellent communication skills, and the ability to work effectively within a multidisciplinary team. Experience in a care or support role is desirable, but full training will be provided. You should be able to demonstrate empathy, resilience, and a commitment to supporting people in acute mental health crisis.


Main duties of the job

As a Support Worker in the CRHTT, you will assist qualified practitioners in delivering intensive, 24 hour support to service users experiencing acute mental health crises, offering an alternative to hospital admission. You will help service users engage with community resources, promote independent living, provide practical and emotional support, and contribute to safeguarding and relapse prevention. The role involves working a rolling rota covering both day and night shifts.

Based within Pennine Care NHS Foundation Trust, you will work alongside a skilled multidisciplinary team, including nurses, practitioners, and external agencies. Detailed job description and main responsibilities

• To assist qualified practitioners in delivering care within the Crisis
Resolution Home Treatment Team.
• To support service users in engaging positively and actively with appropriate services and resources such as benefits, employment,
housing and substance misuse services.
• Be able to identify issues relating to safeguarding adults or children and to ensure first steps to protect vulnerable adults and then to escalate to more senior members of the team.
• To be alert to signs and symptoms of relapse indicators for service users
and to ensure this is communicated effectively to the wider MDT.
• Promote independent living of service user through help, advice, practical and emotional support.
• Provide information and support with health promotion.
• Support the practitioners in monitoring the service user’s physical health and mental state, level of functioning and progress.
• Maintain accurate legible documentation in accordance with Pennine Care NHS Foundation Trust Policies and procedures.
• Provide information and support to enable service users to engage in
meeting education, employment and vocational needs.
• Provide information and support to family and friends of service users

• Promote mental health awareness and reduce stigma and discrimination wherever possible.
• To attend and actively take part in meetings, and training sessions as
required.
• To liaise and communicate across statutory and non-statutory agencies
• To maintain confidentiality at all times.
• To develop knowledge of local resources and opportunities available to
service users and keep an up to date file of community resources which
can be shared with the rest of the team and outside agencies.
• To actively take part in and utilise clinical and management supervision to develop and maintain safe practice.
• To contribute to the learning environment for any students or workers on placement with the team and new staff.
• To take part in the Key Skills Knowledge Framework system to identify
and plan for areas of personal development.

Education/Qualifications


Essential criteria

* NVQ level 3 or equivalent qualification
* Good standard of general education including good numeracy and literacy skills


Experience


Essential criteria

* Experience of undertaking a supportive role.
* Experience of working with individuals with mental health needs
* Experience of working with individuals in distress


Desirable criteria

* Experience of lone working
* Working with groups/ coordinating activities for service users
* Working with people with mental health problems


Knowledge


Essential criteria

* Understanding of safeguarding practice and how to support vulnerable adults to stay safe
* Understanding of signs and symptoms of poor mental health and how to record and escalate as needed
* Understanding of mental health issues and how these impact on individuals.
* Knowledge of physical health observations


Desirable criteria

* Awareness of local services
* Knowledge of benefits / employment systems.
* Ability to work sensitively with people in a mental health crisis
* Understanding and ability to work in partnership with third sector voluntary organisations to promote a range of community-based options for recovery


Skills and Abilities


Essential criteria

* Ability to implement care plans for individuals with the support of others as appropriate.
* Ability to empathise with service users
* Ability to work unsupervised in a range of settings.
* An ability to act calmly in emergencies and to respond in a professional manner to stressful and challenging behaviour


Desirable criteria

* Willingness to engage with or have undertaken Venepuncture and physical health checks training.
