duties, which arise outside normal office hours. The role is to carry out a preliminary assessment of the presenting situation and establish a safe and appropriate intervention pending a follow up by the daytime adult social care services.
Purpose of the Team is to provide a service to individuals and families in social care crisis which requires an immediate and/or urgent response, using social work methods, models, and available resources. The service is operated from 5pm until 8pm, from 4.30pm on Fridays, weekends and public holidays are covered by a Rota basis when social service offices are closed.
Responsibilities and duties of the care co includes OOH staff work autonomously and are responsible for making decisions on behalf of the department in the work covered by the LA Adult Social Care team
1. To receive and prioritise calls from the Emergency Control operators (Lifeline operators or Customer Services)
2. To contact referrers and take details of the referral and decide what action is required.
3. To carry out assessments of need and of risk to clients referred
4. Where it is possible and safe, to deal with the presenting situation by making arrangements that avoid the need for the clients to be removed from the community or their existing placements, including End of Life care, domiciliary care packages and access to health support.
5. Where it is not possible and/or safe to do this, to arrange for the client to be provided with care in hospital, residential home as appropriate.
6. To be responsible for providing an AMHP service within the authority out of hours, including undertaking assessments under the MHA.
7. To respond to the police for an “appropriate adult” service in line with Council policy
The role is on a casual basis and will be required to support the Senior Social Worker working the weekends and Bank Holiday and there could be some occasional evening cover between 5pm - 8pm.
