We are seeking a motivated and compassionate Care Coordinator to join our team at Swan and Forest Surgery. This is an important administrative role, supporting the delivery of proactive, personalised care to our patients, particularly those with long‑term conditions, complex needs, or who require additional support navigating health and care services.
The successful candidate will work closely with GPs, nurses, and the wider multidisciplinary team to ensure patients receive coordinated, timely, and appropriate care. You will act as a central point of contact, helping to organise care plans, follow up on patient needs, and improve overall patient experience and outcomes.
Main duties of the job
* Act as a key point of contact for patients, carers, and healthcare professionals involved in a patients care
* Coordinate and support the delivery of personalised care plans for patients with complex or long‑term conditions
* Proactively identify and follow up with patients who may benefit from additional support or intervention
* Liaise with internal clinical staff and external organisations (e.g. community services, social care, voluntary sector) to ensure seamless care delivery
* Support patients in navigating the healthcare system, including booking appointments, accessing services, and understanding their care plans
* Maintain accurate and up‑to‑date patient records in line with practice protocols and data protection requirements
* Assist with administrative tasks related to care coordination, including audits, recalls, and reporting
* Contribute to multidisciplinary team meetings and support population health management initiatives
* Promote patient independence, wellbeing, and self‑management where appropriate

Key Responsibilities
Care Coordination
* Coordinate and support the development and delivery of personalised care plans
* Identify patients who would benefit from proactive care coordination

Patient Support
* Act as a first point of contact for patients with care coordination needs
* Assist patients in accessing appropriate services

Multidisciplinary Working
* Work collaboratively with GPs, nurses, pharmacists, social prescribers, and other healthcare professionals
