Rate of Pay : £111.0 - £146.28 per day Summary: The Reablement Assistant is a community-based role focused on empowering service users to regain independence and remain in their own homes. Working as part of an integrated team of health and social care professionals, you will implement person-centered support plans that prioritize the dignity, culture, and individual potential of each service user. The role requires high levels of flexibility and a commitment to anti-discriminatory practice across various locations within the city boundary Responsibilities: Assist service users with specific tasks as outlined in their individual support plans Provide personal care, including the safe use of moving and handling equipment. Encourage the development of essential skills for independent living, such as kitchen skills, household maintenance, laundry, and personal health care within a safe environment. Maintain accurate records and report any significant changes in a service users circumstances to the Line Manager. Adhere to all City Council policies, specifically regarding equal opportunities and anti-discriminatory practices. Essentials: Understanding of person-centered care and the "reablement" philosophy of promoting independence rather than over-reliance on support. Experience in assisting individuals with personal care or daily living tasks. Proven experience working effectively within a team environment, preferably alongside health or social care professionals. Experience in teaching or encouraging others to develop practical life skills, such as cooking or household maintenance.
