Whether it be supporting a team member with new skills or attending to a patient and advising their family, your role as a Community Nurse provides you with the opportunity to deliver a high standard of care to patients in their homes alongside developing your leadership skills supporting your team.

Could you be our Community Nurse looking for a new challenge? As a Registered Nurse you will contribute to leading a community nursing team within a multi professional environment.The role requires travel around the local community. You will need to have a full drivers licence and access to a car. The service covers 8am - 8pm working 7 days per week. The shift patterns are 8am – 4pm and 12pm – 8pm fulltime including weekend working.

Main duties of the job

The role includes, wound care, catheter care, palliative care, IV drug administration, medication/ injection administration, diabetes management and many other diverse skills.

There will be an expectation for the successful candidates to work weekends, late shifts and bank holidays pro rata. Our late shift covers until 8pm.

Newly qualified staff will have access to a preceptorship program and a specific clinical upskilling training and PNDs support. Two weeks Trust induction will be provided, plus two weeks’ local team induction were you will shadow your colleagues DNs, TVNs, CNS etc.

As a member of our community multi-disciplinary team, you will work within an integrated team of District Nurses, Specialist Nurses, Community Matrons, Therapists, and Health Care Assistant is to support the provision of high-quality evidence-based care that meets the health care needs of our local community. You will also be required to work in collaboration with Local Authorities, Integrated Care Networks, GP’s, and Acute sector providers to facilitate care pathways that support early discharge and prevent hospital admission.

Qualifications and Training

Essential criteria

1. Registered General Nurse, Adult Branch and current NMC Registration
2. Experience and evidence of teaching and assessing e.g. Preceptorship/Mentor or working towards
3. Commitment to complete advanced physical assessment course/clinical decision making/District Nursing Specialist Practitioner Course
4. Evidence of recent CPD
5. Ability to work in community setting/home visits

Desirable criteria

6. Advanced clinical skills - ability to assess, diagnose and treat as an independent practitioner minimum level 6 study or similar
7. Advanced clinical assessment skills, clinical decision making/District Nursing Specialist Practitioner Course at minimum level 6 study or similar course or commitment to complete.

Experience

Essential criteria

8. Demonstrable experience and work qualification, preferably within community care setting.
9. Experience of supervising junior staff in the team or work towards developing.
10. To have an interest in audits and clinical governance.
11. Experience of and ability to communicate effectively with variety of partners and stakeholders - verbal, written and electronically
12. Ability to problem solves and prioritise.

Desirable criteria

13. Evidence of a range of clinical competencies appropriate to the nurse role e.g. venepuncture, palliative care, IV therapy, wound care, diabetes management.

Knowledge

14. Demonstrates an understanding of ECP Locality Services.
15. Aware of current professional nursing issues.
16. Demonstrates sound application of clinical knowledge
17. Be able to critically analyse research and use this to substantiate best practice
