Job summary

Whitewater Health is seeking an experienced and highly motivated Advanced Clinical Practitioner (ACP) to join our dynamic and supportive team. This is an opportunity to work in a forward-thinking and patient-centred GP practice, committed to delivering high-quality care within the local community. We are rapidly evolving with implementation of new technology into clinical practice to enhance patient care and reduce admin burden. You will be joining our team of supportive Advanced Clinical Practitioners who work alongside our full GP team.

We are seeking an additional ACP to join our team on a 32-hour contract, you must be able to work a Monday and Tuesday.

Main duties of the job

The successful candidate will be a qualified Advanced Clinical Practitioner and hold a Digital Badge from the Centre for Advancing Practice. You will work autonomously, providing expert clinical assessment, diagnosis, treatment, and management of patients presenting with undifferentiated conditions in a primary care setting.

You will be expected to manage acute, urgent, and chronic presentations from both adults and children, demonstrating advanced clinical decision-making, critical thinking, and clinical reasoning skills.
Key Responsibilities

* Undertake comprehensive clinical assessments of patients presenting with a wide range of undifferentiated conditions.
* Diagnose and initiate appropriate care, including prescribing and referrals, in line with current guidelines and best practice.
* Work collaboratively within a multidisciplinary team to ensure holistic and seamless care for patients.
* Contribute to the ongoing development of clinical services and protocols within the practice.
* Provide clinical leadership and mentorship to colleagues where appropriate.
Essential Criteria
* MSc in Advanced Clinical Practice or equivalent qualification.
* Digital Badge awarded by the Centre for Advancing Practice.
* Independent prescriber (V300).
* Significant experience working in Primary Care.
* Proven ability to manage undifferentiated presentations in both adult and paediatric populations.
* Excellent communication, organisational, and interpersonal skills.

There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a.Practise in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice

b.Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information

c.Act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change

d.Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate

e.Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make a positive change

f.Use expertise and decision-making skills to inform clinical reasoning

g.Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care

h.Exercise professional judgement to manage risk

i.Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

j.Implement and evaluate individual specialised treatment plans for chronic disease patients

k.Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

l.Be a professional role model for the organisation

m.Actively engage in peer review to inform own and others practice, formulating and implementing strategies to act on learning and make improvements

n.Actively seek feedback and involvement from individuals, families, carers, communities and colleagues in the co-production of service improvements

o.Critically assess and address own learning needs, negotiating a personal development plan that reflects the breadth of ongoing professional development across the four pillars of advanced clinical practice (clinical practice, leadership and management, education and research).

p.Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

q.Maintain accurate clinical records in conjunction with extant legislation

and to ensure SNOMED CT codes are used effectively

r.Develop, implement and embed an effective call/recall system

s.Critically appraise and synthesise the outcome of relevant research, evaluation and audit, using the results to underpin own practice and to inform that of others

t.Develop and implement robust governance systems and contribute to practice targets (QOF, etc.), complying with local and regional guidance

u.Chaperone patients where necessary

v.Support the team in dealing with clinical emergencies

w.Support the team with all safeguarding matters in accordance with local and national policies

x.Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

y.Deliver opportunistic health promotion where appropriate

Inaddition to the primary responsibilities, the Advanced Clinical Practitionermay be requested to:

a.Supportthe IPC lead, ensuring compliance, and undertaking audit as necessary

b.Monitorand ensure the safe storage, rotation and disposal of medicines

c.Undertakeoverall management of a clinical team, providing guidance when necessary,acting as a mentor to students and newly qualified staff

d.Maintainan effective clinical staff rota ensuring all clinics are staffed appropriately

e.Participatein local initiatives to enhance service delivery and patient care

f.Supportand participate in shared learning within the organisation
