Overview

You will have opportunities to work closely with multidisciplinary teams in the community, primary care, the acute neuro rehab team, the stroke early supported discharge team and other external agencies. The post is within a multidisciplinary team and you will work alongside Neuro Physiotherapists, Occupational Therapists, Psychologists, Speech and Language Therapists and Rehab Assistants. The post is based at Hinchingbrooke Hospital, Huntingdon with potential to cover across the South of the county and county-wide duties as required. You will work in an agile manner using virtual consultation to manage your caseload, and you will have access to a smart phone and laptop. You will be required to travel independently around the county to meet strict deadlines.

We are seeking a dynamic Band 4 Rehabilitation Assistant to support neuro rehab as in-reach to inpatient rehab beds and with patients in their own homes. You will support patients with physical, cognitive and psychosocial challenges as part of a MDT to achieve their potential and improve health, care and outcomes.

The ideal candidate will be flexible and adaptable to support service delivery across the organisation. A personal car is required for independent travel across the work locality.

For this role you will be required to travel independently around the county and hold a full UK driving licence with use of a vehicle. * To work autonomously in the community setting under the direction of a registered practitioner within the integrated multi-professional ESD and neuro rehab team.
* To work in a ward environment to support patients on the ESD pathway, collaborating with the ward MDT and the ESD/CNRS.
* To assess patients’ physical and functional needs, plan and implement therapy interventions for patients with stroke and neurological problems.
* To use clinical judgement to evaluate patient progress and record objective and subjective measurements.
* To manage your own caseload of patients with ongoing complex needs and plan/prioritise your workload.
* To undertake duties delegated and supervised by a registered professional following prescribed care packages.
* To keep the patient at the centre of care, ensuring patient and carer participation in decision making.
