Community Mental Health Care Coordinator

Ensure the mental health needs of adults with mental illness referred to Hillingdon CMHH are appropriately assessed, supported and a high level of quality care is delivered within available resources.

Provide support and advice to users, carers and other professionals on mental health issues.


Main Duties

* Manage a caseload, medication administration, duty work and attendance to MDT meetings.
* Undertake holistic assessments of service users using the integrated notes system (SystmOne), incorporating the Single Assessment Process, risk assessment and mental state examinations.
* Promote the work of Hillingdon CMHH and raise awareness of mental health issues affecting adults with mental illness.
* Maintain up‑to‑date clinical records and documentation (including electronic records) in line with local policies, NMC guidelines and CNWL policy.
* Ensure own professional practice and clinical knowledge is maintained at all times in accordance with NMC guidelines and CNWL policies and procedures.
* Work in partnership with the primary care team, statutory and voluntary agencies to ensure appropriate care needs are met.


Job Responsibilities

* Respond to duty referrals daily, prioritise urgent referrals.
* Maintain patient safety in collaboration with patients, carers and team members.
* Recognise signs of patient harm, abuse or neglect and take reasonable steps to protect patients.
* Provide clinical advice on complex issues to other members of the clinical team and staff from other disciplines.
* Undertake urgent and priority assessments, including comprehensive risk assessments.
* Make decision‑making and coordination during crisis situations.
* Support service users short term while Care Coordinator / Lead Professional is absent or requires an urgent response.
* Liaise with other services such as Police, A & E, Single Point of Access.
* Participate in setting quality standards and offer a consistent approach to people within the team.
* Maintain an effective reporting standard, verbally and in writing on patient conditions.
* Support team members with specialist assessments, e.g., Mental Capacity Act 2005, Deprivation of Liberty Safeguards, s42 Care Act, Safeguarding Adults, Making Safeguarding Personal.
* Maintain professional standards of care at all times.
* Supervise junior staff members and support students on placement.
* Undertake and maintain all essential training for the role.

We are service‑user led and work with adults experiencing mental and physical health issues requiring a multidisciplinary approach. After a recent transformation to the new Hub model, we offer an exciting opportunity to join a team adapting to more responsive services.

We are a multidisciplinary service comprising mental health nurses, occupational therapists, psychiatrists, psychologists, employment specialists and peer support workers. The team is supported by a strong administrative system and offers an excellent work environment with good links to local statutory and non‑statutory services.

We provide regular individual supervision and offer flexible working arrangements, including remote work opportunities.


Person Specification


Education and Qualifications

* Current professional registration as a Registered Mental Health Nurse, Occupational Therapist or Social Worker.


Experience

* Experience working within the community setting.
* Experience supervising the work of others.


Skills and Knowledge

* Ability to participate in MDT work and contribute to the team.
* Knowledge of therapeutic assessment and interventions to reduce distress for individuals in acute mental health crises or ongoing support needs (risk assessment, MSE, capacity assessment, full needs assessments) and manage a complex caseload effectively.
* Experience with standard setting and audit.
* Ability to teach and supervise qualified and unqualified staff.
* Ability to assess and support individuals experiencing acute mental illness distress and crisis.
* Strong verbal, non‑verbal communication and telephone skills.
* Skills in a range of therapeutic interventions and psycho‑social treatment options.
* Demonstrated clinical leadership.

This position is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and requires a submission for Disclosure to the Disclosure and Barring Service (formerly CRB) to check for any previous criminal convictions.
