Band 6 Mental Health Practitioner - North Somerset
Primary Care focus: Contribute to delivering and co-developing the primary care mental health service model. This includes care pathways for integrated physical and mental health care within a designated Primary Care Network. Be the point of contact for patients referred from primary care, promote early assessment/treatment, and foster relationships with community-based services.
Work closely with the primary care team, including social prescribers and recovery navigators, to support adults (18+) with mental health needs, especially where primary care is the most appropriate setting. Collaborate with specialist community mental health services to ensure smooth transitions and facilitate access to evidence-based interventions.
Connect individuals to community and voluntary sector support, working alongside Social Prescribers & Wellbeing Coaches. Facilitate mental health assessments and support primary care in accessing appropriate services. Build links between Primary Care Networks, Community Mental Health Teams, Social Care, and voluntary organizations to enable seamless transitions and support.
Promote understanding and confidence in mental health care delivery and collaboration among all involved agencies. Provide learning opportunities for primary care staff to enhance their mental health knowledge and skills. Offer information, signposting, and support to service users and their families or carers.
Develop strong, trusting relationships with referrers and primary care professionals, underpinning a solutions-focused approach. Facilitate safe and effective mental health practices within primary care, including training and supervising junior staff where needed.
Clinical Practice:
* Work with strengths and aspirations of individuals.
* Provide formulations and recommendations, including risk management advice.
* Assess cultural and diversity impacts with colleagues.
* Advise on conditions for people with functional and organic mental health needs.
* Support family and carer needs, including third sector agencies.
* Evaluate risks positively and support safeguarding procedures.
* Respond promptly to contacts and ensure seamless transfer to specialist services when needed.
* Follow up with patients post-assessment as appropriate.
* Contribute to multidisciplinary team processes with evidence-based expertise.
* Promote collaboration and positive attitudes among all stakeholders.
Maintain professional development, adhere to professional codes, and ensure safety and health standards. Keep accurate health records in line with standards.
