Job overview
As part of the NHS Community Mental Health Transformation Programme, an exciting opportunity has arisen for a Band 7 Mental Health Nurse Practitioner to work as part of the Community Urgent Care Response Teams in Primary Care, offering specialist Mental Health support to patients cared for within the community. The post holder will work closely with Physical Health Urgent Care Team colleagues in Primary Care, providing advice and support for patients with complex or challenging Mental Health needs, and where appropriate, provide initial assessment and first-line treatment followed by signposting, including direct access to support from Older Adults Enhanced Community Teams.
The post holder will provide a high standard of assessment, including risk management and treatment, for patients referred to the service, including those with complex mental health needs.
Main duties of the job
* Exercise a high degree of professional autonomy and critical judgment, making autonomous decisions using knowledge, skills, and experience, and maintaining competencies via supervision.
* Provide specialist mental health advice and guidance to health professionals and colleagues, including care homes and patients within Physical Health Virtual Wards, offering expert clinical support, initial assessments, and first-line treatment.
* Offer education and training to professionals, families, and carers to improve health outcomes and provide care closer to home.
* Undertake Independent Non-Medical Prescribing where clinically appropriate, utilizing NICE Guidance and Trust prescribing policy, or be willing to undertake NMP training once in post.

* Undertake Independent Non-Medical Prescribing, where appropriate, using NICE Guidance and Trust policies.
* Collaborate with Physical Health Urgent Care Teams in Primary Care to support patients with complex mental health needs, providing assessments and initial treatments, and signposting to support services.
* Support care planning and provide education and practice development for referrers.
* Deliver education and training to professionals, families, and carers about Older Adult Mental Health to improve outcomes and facilitate care closer to home.
* Provide clinical leadership, offering specialist mental health advice and guidance, initiating change based on audits and evidence-based research.
* Conduct risk and other assessments, ensuring timely documentation in accordance with Trust policies.
* Develop coordinated links within the Trust and with external health and social care agencies to meet patient and family needs during referral, assessment, and signposting phases.
Person specification
* Registered Mental Nurse (RMN)
* Postgraduate experience in Older Adult Mental Health Community Services
* Specific training/experience in Older Adult Mental Health
* Effective risk assessment and management skills
* Advanced knowledge of current best practices in Older Adult Mental Health assessment
* Ability to build and maintain effective relationships with teams and partner agencies
* Ability to work independently and as part of a team
