We are looking to employ a receptionist/care navigator to join our existing team. This is a permanent role working mainly at our Matlock practice.
Main duties of the job
You will help to manage the needs of our patients through answering queries, making, and managing appointments within our reception, and ensuring that people have good quality written or verbal information to help them make choices about their care. Also answer queries from other agencies and hospitals.
You will raise awareness when identifying patients who may benefit from support and signpost them appropriately. Work sensitively when interacting with patients, their families, and carers to capture key information.
Keep accurate and up-to-date records of contacts, and update missing or out-of-date information about the persons circumstances appropriately using GP and other records systems relevant to the role, adhering to information governance and data protection legislation.
Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, equality, diversity and inclusion training and health and safety.
Demonstrate a flexible attitude and be prepared to cover for holiday and sickness within our patient services team. There will be a requirement to work at both Matlock and on occasions at Ashover too. Full training will be given with regular reviews.
This role is approx. 15 to 20 hours per week over a minimum of three days.

Matlock and Ashover Practice has 9850 patients. We have two sites one in Matlock and one in Ashover. The practice in Ashover is a dispensing practice.
