Overview:
As a Health Coach, you will play a vital role in promoting health and wellbeing for
individuals aged 50+ across Norwich and Norfolk. This includes supporting clients at
home, online, and in community settings to improve physical activity levels, build
confidence, and access local services and social opportunities. This role is part of Age UK
Norwich's integrated community health service and contributes to preventative healthcare
by facilitating healthier, independent living. You will be part of the Service Delivery Team
and report directly to the Service Delivery Lead.
Our Health coaches are at the heart of prevention. You will support older people one-to-one co-ordinate exercise classes in the community to improve physical activity, build confidence, and re-engage with community life. Whether supporting someone recovering from illness, tackling frailty, or simply looking to stay well, you will deliver safe and entertaining sessions to get people back on their feet.

Key responsibilities:
* Deliver tailored 1-to-1 health coaching via home visits, telephone, online platforms, or community-based sessions.
* Facilitate group exercise and wellbeing activities to support physical and mental health goals.
* Support clients with building confidence, reducing isolation and engaging in social and community networks.
* Identify and empower 'Community Champions' to support peer-led groups, share skills and sustain engagement.
* Build and maintain relationships with community partners, health professionals and stakeholders to enhance service reach.
* Represent Age UK Norwich at events and forums to promote our heath and community support work.
* Conduct client feedback surveys and contribute to service evaluation and continuous improvements efforts.
* Work flexibly as part of a multidisciplinary team, contributing to group sessions, peer learning activities, volunteer support and cover during absences.
* Help with outreach, education and promotion by collaborating with the Marketing and Communications team across various platforms and events.
